Job Description

As an outpatient physical therapist, you'll provide PT services to help patients regain their health and mobility. Working at an outpatient facility, rehabilitation center, private practice or physical therapy office, you'll assess patients' mobility, strength and function, develop treatment plans, maintain records and provide patient education. You'll play a critical role in helping patients develop strength, reduce pain and improve function.

Minimum Requirements

* At least 1 year of recent experience in an outpatient setting and specialty
* Degree in relevant field
* BLS Certification (AHA)
* Current state/compact license

Orientation & Training:

TMC offers an orientation and flatbed training program just for you! We've been training apprentice drivers for nearly 30 years.

Our student driver training program is recognized as one of the best in the country. It is also one of the few on-the-job truck driver training programs approved by the Department of Labor as a certified Heavy Truck Driver Apprenticeship program! Apprentice drivers are paid $650 per week during orientation and OTR training with one of our certified flatbed driver trainers!

- Home time is important to us, so we try our best to pair you with a trainer that lives within 150 miles of your home so you can enjoy your weekends with friends or family

Become the Case Management Director you've always aspired to be

- Assume responsibility for the day-to-day operations and human resource management of the Case Management department.
- Oversee the interdisciplinary plan of care and the discharge planning process to ensure the effectiveness and appropriateness of services with a central focus on census management, patient care outcomes, and key care indicators.
- Act as a patient and family advocate, ensuring that services are delivered to meet the needs of patients and their families.

Qualifications

- Current CCM® or ACMTM certification is preferred.
- Must be qualified to independently complete an assessment within the scope of practice of his/her discipline.

- For Nursing, must possess bachelor's degree in nursing (BSN) with RN licensure.
- For other eligible health care professionals, must possess a minimum of a bachelor's degree; a graduate degree is preferred.
- Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience.
- May be required to work weekdays and/or weekends, evenings and/or night shifts.
- May be required to work on religious and/or legal holidays on scheduled days/shifts.
