Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical capabilities (e.g., lifting, assisting lifting, standing, etc.)

Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field; and

• At least two (2) years of experience working with the I/DD population and/or behavioral health needs; or

• Satisfactory combination of education, experience and/or training as authorized by START/NCSS and/or appropriate State regulatory agency (e.g. OPWDD, etc.); however, all START Coordinators must have at least a bachelor’s degree in human services discipline.

• Successful completion of START Coordinator Certification training within 12 months of appointment, including demonstration of competencies in all associated areas.

• Ability to provide on-call coverage for crisis services on a regularly recurring basis, as designated by START leadership in assigned region beyond regular work hours including weekdays, weekends, evenings and/or overnight on a 24/7 basis, as required.

• Ability to travel regularly across assigned region(s) to conduct in-person assessments, evaluations and visits as required by caseload and to occasionally travel across other regions within state of operations and/or to other states as needed for NCSS meetings or events.

What you’ll do:

During a catastrophe, or post event, performs property reinspections for catastrophe claims with a minimum of 28 consecutive days during the catastrophe event or post event.   • Ensures risks associated with business activities are effectively identified, measured, monitored, and controlled in accordance with risk and compliance policies and procedures.

University Health is seeking a passionate, board-certified Nurse Practitioner or Physician Assistant to join our dynamic Nephrology team. In this hybrid role—split between inpatient and outpatient settings—you’ll provide compassionate, patient-centered care while helping advance our commitment to excellence in nephrology.

The ideal candidate is a dedicated clinician who thrives in a collaborative environment, values lifelong learning, and is eager to support a diverse patient community. Two or more years of direct nephrology experience are preferred.

Education & Experience

Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.

- At least 3 years of nursing experience with 2 years of advanced practice preferred.

Licensure & Certifications

Current Texas RN and APRN license (for NPs) or Texas PA license. Must hold national certification, prescriptive authority, and AHA BLS (or higher) certification as required by Medical-Dental Staff Bylaws. A valid DEA number is required within 90 days of hire.

Position Overview
The RN Clinical Management Specialist is a qualified professional responsible for collaboration with Home Health and Hospice leadership in planning, coordinating, and delivering quality services to the client. This is a regional resource assigned as needed for interim clinical leadership, training, and/or clinical support.

This position is a hybrid role that will require in office training and clinical support in any of the locations in Florida. You must live near one of the locations.

Requires weekly travel to the potential locations:

Sarasota, Ft. Myers, Naples, Miami, Stuart, Fort Lauderdale, and Port Charlotte.

Essential Job Functions

- Monitors and assesses company electronic medical records (EMR) workflows and assists as needed or assigned
- Provides support with the intake process and admission decisions
- Facilitates virtual training for newly hired/rehired/promoted Clinical Managers and Nurses
- Serve as interim Clinical Manager when position is vacant
- Act as mentor for new Clinical Managers

Requirements

- Registered Nurse with an active and unencumbered license in the state of residence
- Expertise and experience with the Homecare Homebase (HCHB) EMR system
- Three (3) years of clinical experience within the last five (5) years in a home health agency, home care setting, or health facility
- Homecare Homebase (HCHB) & Oasis experience is required
- Valid and acceptable driver’s license with proof of automobile insurance coverage
- Travel by car between regional offices will be required
