Position Overview:This full-time role is open to Nurse Practitioners (FNP, AGACNP, ACNP, AGPCNP) and Physician Assistants passionate about post-acute care.
You???ll work with an interdisciplinary team, ensuring smooth care transitions while collaborating closely with hospice, palliative, and home care agencies.

Provides direct patient care services.
Elicits detailed patient histories, assesses patient medical needs, initiates appropriate diagnostic testing and/or treatment and completes appropriate medical record documentation.
Screens patients for appropriateness for cardiac testing.
Supervises exercise and pharmacological stress tests.
Make use of evidence-based medicine procedures by utilizing quality and cost-effective care Performs other procedure related tasks in Cardiology.
