In the Patient Care Manager RN Hybrid role, you are responsible for the supervision and coordination of clinical services and provide and direct provisions of nursing care to patients in their homes as prescribed by the physician. * Completes comprehensive assessments (OASIS), medication reconciliation, and initial/comprehensive nursing evaluation visits.

Strong organizational skills with the ability to prioritize tasks.
Strong problem-solving skills and ability to provide options.
Demonstrated computer experience with solid keyboarding skills and proficiency with current software packages.
