Unit Description: Inpatient Rehabilitation nursing is a different type of nursing. We are primarily coaching and educating our patients as we care for their medical needs.Β  We strive to help our patient’s transition back home by helping to guide, coach and direct them to become independent in their activities of daily living. We work very closely with PTs and OTs and MD’s and we help our patients carry out their treatments.Β  If you like to develop relationships with your patients and their families, and you have patience and good people skills, inpatient rehabilitation may be the right kind of nursing for you.Β  We are a 35-bed Inpatient Rehabilitation Unit located at the Fanny Allen Campus where Fletcher Allen’s Rehabilitation Center is based. We provide team-based care, working closely with PTs, OTs, SLPs, Recreational Therapists, MD’s and Medical Psychologists.Β We have a wide range of patients, including brain injuries, strokes, spinal cord injuries, and multi-traumas or illness that requires a more intensive Rehabilitation for patients. Our average length of stay is 13 days, but our range is 5 to 90 days.Β  Around 95% of our patients return home with a small present needing to go to a SAR (Suba acute Rehabilitation) or to a long-term care facility.

On-call: Not required

Department Description: Our High Technology Nursing program provides skilled, compassionate in-home nursing for patients who require around-the-clock care and advanced medical technology. What You’ll Do

Department Description: The Community Hospice team delivers care to patients in the community, providing a wide range of skilled nursing care and support. We are an interdisciplinary team all collaborating together to support patients to remain in the setting of their choice, their home. The RN will provide a full range of skilled nursing care to patients in a home care setting with a focus on patient education and palliative care to meet the physical and emotional needs of patients, while educating and providing support to families.

Schedule is every Fri/Sat/Sun/Mon from 8am to 6pm

Minimum Requirements

- Associate's degree in nursing or higher.

- Minimum of three years of experience as a Community Hospice Nurse or minimum of four years of experience in home health nursing.

- Certification in hospice and palliative care preferred.

On-call: Not Required.

Driver Rehabilitation Specialist:

Driver Rehabilitation at the University of Vermont Medical Center is comprised of a small team of multi-talented occupational therapists who are Vermont Driving School Instructors and Certified Driver Rehabilitation Specialists.

Driver Rehabilitation Specialists provide thorough evaluations and appropriate treatments for people with conditions that may affect their ability to drive safely. They offer specialized driving programs for clients who are new or potential drivers (students), clients who have low vision, senior drivers, and drivers who require adaptive equipment. These services are provided in our clinic as well as in our two adaptable driver training vehicles.

The Occupational Therapist/Driver Rehabilitation Specialist

  • Provides direct patient care including evaluations, consultations, and treatment programs based on functional limitations and patient needs.
  • Designs, implements, and modifies therapeutic interventions that may include, but are not limited to, therapeutic exercise, behind the wheel training, adaptive equipment prescription training and education and other patient-related education (related to driving and community mobility skills).
  • Educates patients on topics including current diagnosis and implications for driving.
  • Communicates regularly with a variety of healthcare professionals, including physicians, and case managers and other regulatory entities including the DMV to ensure successful patient treatment outcomes and carry through of driving recommendations.
  • Responsible for the ongoing development of clinic skills through continuing education programs, professional contacts, journal articles, and participating in professional organizations
  • Participates in clinical staff education, meetings and quality improvement projects that support current best practice.
  • Perform daily documentation in accordance with UVMMC standards. Documentation is completed in a timely manner consistent with all laws and regulatory requirements.
  • Manages caseload effectively and uses excellent time management, organizational, interpersonal and communication skills
  • Maintains complete and accurate billing practices. Assists front desk in maintaining insurance authorizations.
  • Protects patients’ rights by maintaining confidentiality of personal and financial information
  • Maintains CDRS status by meeting the continuing education requirements as outlined by ADED and by keeping membership up to date
  • Maintains Vermont state Driver Instructor Certification status by meeting the continuing education requirements and renewal fees

EDUCATION:

Bachelors of Science (or higher) degree from an Accredited Occupational Therapy program. Licensed or eligible to be licensed to practice occupational therapy in the state of Vermont.

Preferred: Certified Driver Rehabilitation Specialist (CDRS) and Vermont Driving School Instructor License preferred, but not required. If not currently certified/licensed, must be completed within 2 years of hire.

EXPERIENCE:

Minimum of 3-5 years of experience is ideal.

The occupational therapist collaborates with the rehabilitation team to develop a comprehensive plan of care to prepare the patients for a successful discharge. Department Description: Want to have every other week off? We are seeking a full time On Call Visit RN to work closely with the triage nurse team to support clients in the home setting from 5pm to 8am. The Community Hospice team delivers care to patients in the community, providing a wide range of skilled nursing care and support. We are an interdisciplinary team all collaborating together to support patients to remain in the setting of their choice, their home. The first 5 hours you will have intentional work such as admissions, the rest of the time you just need to be available for any urgent calls!

RN II and RN IIIs are considered for this position.

Minimum Requirements:

- Associate's degree in nursing or higher.

- Minimum of four years of experience as Community Hospice Nurse or minimum of five years of experience in home health nursing.

- Candidate must live within reasonable driving distance to our service area as you will be making on-site visits at patient's homes in Chittenden and Grand Isle counties.

- Certification in hospice and palliative care preferred.

On-call: Required.

Incentives: $10,000 Sign On Bonus for external applicants. Evening and Night Differentials range from $2-$5.20! 7 nights on / 7 nights off!
