• Assess and monitor the health of postpartum mothers and newborns, identifying changes, addressing complications, and recording significant conditions and reactions
• Assist with and educate on breastfeeding, addressing challenges to ensure successful feeding practices
• Perform newborn assessments, including vital signs, weight checks, and required screening tests
• Respond to life-saving situations using evidence-based protocols and best practices
• Collaborate with interdisciplinary teams
• Educate new parents on infant care, maternal recovery, and postpartum health, including safe sleep practices and early signs of complications

In the Patient Care Manager RN Hybrid role, you are responsible for the supervision and coordination of clinical services and provide and direct provisions of nursing care to patients in their homes as prescribed by the physician. Primary Responsibilities:

* Directly/indirectly supervises home health aides and LPNs, provides instruction, and assigns tasks

* Completes comprehensive assessments (OASIS), medication reconciliation, and initial/comprehensive nursing evaluation visits.

* Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals

* Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate

Essential Duties and Responsibilities

• Deliver outstanding customer experiences by responding promptly, accurately, and professionally across all communication channelsstriving for single-contact resolution whenever possible.

• Perform compliant policy administration, including processing policy changes, updating contact information, and setting up payment plans in accordance with state statutes and regulatory requirements.

• Maintain timely workflow management, ensuring Level I tasks are processed within established service level expectations and keeping teammates and leaders informed of progress.

• Promote and practice responsible use of company resources, aligning actions with company policies and efficiency goals.

• Engage in ongoing learning, including training on workers compensation insurance, company systems, and industry best practices.
• Work effectively in a remote-first, hybrid environment, meeting availability expectations and demonstrating reliable, professional communication.

Qualifications

• Education: Bachelor's degree preferred (or a combination of education and relevant experience).

• Certifications: Registered Workers Compensation Specialist (RWCS) designation required within 12 months (provided by MEM).

• Licenses: Valid driver's license required.

• Experience: 13 years in a customer service role; insurance experience preferred.
