• Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors.
• Coaching for health: The PCP acts as a health coach, rather than just a consultant for sickness, by helping patients set short and long-term health goals, partners with the patient to work toward the goals, and frequently follows up on those goals on the path to improved health for their patients.
• Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient in helping them achieve their goals, and when leading their care teams towards their performance goals.
We are an outcomes-focused, value-based organization and for their panel of patients, the following metrics are regularly measured to help PCP’s become successful and reach partnership status: patient admissions/thousand, using between 18-21 appointment slots per day (each new patient count for 2 slots, follow-up patients 1 slot), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures. Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams. Culture is very important in the medical centers and because PCP’s are leaders in our organization and centers, they are expected to help champion a positive culture of love, accountability, and passion along with center leadership.
The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.
The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
- The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
- The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
- It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
- The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
- The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
- For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
- PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
- Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
- Performs other duties as assigned and modified at manager’s discretion.

- Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes.
- Fluency in English, verbal and written. There may be jobs in some centers that require fluency in other languages, and this will be made known at the time of application.

EDUCATION AND EXPERIENCE CRITERIA:
MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required
Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required
Must have a current DEA number for schedule II-V controlled substances
Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment.
PAY RANGE:
$221,141 - $315,915 SalaryThe posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for a bonuses or commissions.

Job Summary: The Construction Project Manager will coordinate and control the construction and renovation of convenience store facilities. Provide project management and coordination of outside contractors for the successful renovation and construction of convenience stores.
Essential Functions:
- Lead and manage all phases of construction and renovation projects for convenience store facilities.
- Ensure projects meet company standards for quality, safety, budget, schedule, design intent, and code compliance.
- Oversee contractor and subcontractor selection, qualification review, and performance management.
- Monitor and control construction budgets, expenditures, and invoice approvals.
- Coordinate procurement and scheduling of materials, labor, and company-supplied equipment.
- Obtain and manage required permits, approvals, and regulatory documentation.
- Conduct regular site visits to ensure adherence to timelines, specifications, and company standards.
- Facilitate communication among internal stakeholders, vendors, contractors, and design professionals.

Reporting to the Director, Content and Growth Marketing, this team member is both a strategic and creative thinker who obsesses over the details, has a passion for storytelling, and has proven success in building a social presence for luxury brands. This position is located on-site at our Corporate Headquarters in Boston Heights, OH.
Key Responsibilities
- Develop and execute Arhaus’ social media strategy across platforms including Instagram, TikTok, Pinterest, Facebook, YouTube, and emerging channels
- Own the social content calendar, aligning with seasonal campaigns, product launches, and brand initiatives
- Partner with Creative, Content, PR, Merchandising, and Digital teams to advocate for and develop compelling, on-brand social content
- Manage day-to-day publishing, community engagement, and brand voice across channels
- Identify and collaborate with creators to create content aligned with Arhaus’ aesthetic and values
- Monitor social trends and cultural moments to keep the brand relevant and inspiring
- Track, analyze, and report on social performance, providing insights and recommendations for optimization
- Manage social media tools, workflows, and best practices
Required Experience
- Bachelors degree in Marketing, Communications, Design, or a related field
- 4-6 years of experience in managing social media for a consumer, lifestyle, or luxury brand
- 2-3 years in managing direct reports, supporting the growth of a team
- Strong understanding of social platforms, algorithms, and best practices
- Proven ability to develop visually compelling, brand-forward content
- Experience with social media management and analytics tools (e.g., Dash Social, and other native platform tools)
- Excellent written communication skills and an elevated editorial sensibility
- Strong organizational skills with the ability to manage multiple projects and deadlines
- Experience managing content creator programs that fuel social growth
Preferred Experience
- Experience in luxury, home furnishings, fashion, or design-driven brands
- On-camera, video editing, or content production experience
- Familiarity with paid social and growth marketing channels

The Graphic Designer of Catalog will support the Creative Team in translating art direction into effective and highly creative photo driven layouts for seasonal catalogs and additional consumer and internal facing projects. You will use your passion for elevated design, photography, and Photoshop to create beautiful layouts, and to reimagine interior spaces. You bring a passion for brainstorming and collaborating on projects in a team-based atmosphere and the proven ability to own designs from start to finish.
Key Responsibilities
- Creating multiple catalog layout solutions that explore different expressions of elevated Typesetting and photographic compositions, to present options for final layout selection.
- Photoshopping images to align photography with the Creative Director’s vision for art direction.
- Support the Creative Director’s vision by creating mood boards including gathering inspiration for interior design direction, photo art direction, floral direction, layout direction, color direction, furniture direction, location direction, and set direction.
- Preparing presentations for catalog concept reviews and catalog pagination reviews.
- Manage multiple design projects, and other design deliverables simultaneously.
- Participate in design reviews with a range of stakeholders across the organization.
- Drive design projects through all creative phases, from concept, to photoshoot planning, to design reviews, to passing off final files.
- Responsible for presenting your work to the team, routing files for approval, and releasing organized files to production partners when approved.
- Maintain accountability for overall quality of creative work.
- Partner with Marketing leads to explore designs that elevate the brand and excite customers.
Experience/Requirements
- Creative portfolio showing your superior understanding of Photoshop with multiple visual examples of different Photoshop techniques.
- Graphic design portfolio demonstrating photo driven layouts and print-based design solutions with an elevated aesthetic.
- Exceptional creative concepting ability, as evidenced by a diverse portfolio of graphic design projects.
- Eye for visual storytelling with elevated photography, typesetting and graphic design.
- Experience ensuring designs translate to printed pieces.
- Organized and able to prioritize, multi-task, and work independently through ambiguity.
- Experience creating photo driven designs and maintaining consistent typographic standards.
- Strong project management skills as an individual contributor and the ability to influence others as a project leader.
- Able to work well in cross-functional teams.
- Excellent presentation, collaboration, and communication skills, as well as a strong customer service mentality, and ability to build and maintain relationships.
- Visual design expertise demonstrated through mockups, prototypes, and style guides.
- Fluent in English, with excellent communication, presentation, and social skills.
- Strong communication, presentation, and interpersonal skills including the ability to communicate design concepts appropriately to different audiences in varying levels of the organization.
- Bachelor’s Degree in Advertising, Design, or other relevant field.
- Fluent in Photoshop, InDesign, and Adobe Creative Suite.
- Passion for Interior design, creative drawing and painting with examples of your work is a plus.

* Spinal Cord Injury (SCI)
Primary skills utilized/learned:
* Ventilator/Trach Care
* SCI retraining (Bowel & Bladder management program)
* Wound prevention and care
* Specialty treatment and technology for regain of function
* Rehabilitation specific education for patient/family
Personnel RN will work with:
* LPNs, Patient Care Nursing Assistants (PCNAs), Nurse Interns, Providers, Social Workers/Case Management, Respiratory Therapy, PT/OT, Speech Therapy, Art & Music Therapy, Dieticians
Number of beds on unit:
* 24 beds
