Physician Assistant / Physician Associate (PA-C) - Primary Care - Concierge Medicine
Join a growing family medicine practice and be the primary provider for our Lufkin clinic. This is a concierge medicine practice that allows for a patient panel with longer visits, relationship-based care, and a focus on prevention, functional medicine, and whole-person health. We're looking for someone that enjoys working as part of team, but is also comfortable practicing as an autonomous provider. You'll be the face of this clinic with a physician working on site a couple days a week.
What You?ll Do
- Serve as the lead Physician Assistant, providing comprehensive primary care in person and via telehealth.
- Practice across family medicine, functional medicine, medical weight loss, hormone optimization, and some light urgent care.
- Perform basic procedures. Will train if needed (e.g., suturing, splinting).
- Collaborate closely with an experienced physician team to ensure outstanding outcomes and continuity of care.
- Build lasting patient relationships with adequate visit time and a patient-first approach.
Qualifications
- Current, unrestricted Texas Physician Assistant license.
- Certified as a PA
- Will train the right provider.
- Confident independent clinician with excellent assessment, diagnostic, and procedural skills.
- Comfortable with telehealth and modern EHR workflows.
- Superb communication and interpersonal skills.
- Strong alignment with a patient-centered mindset.
- Able to perform (or eager to learn) basic procedures such as suturing and splinting.
- Experience with functional medicine is preferred but not required.
Compensation & Benefits
- Base salary: $125,000?$150,000 (based on experience).
- Quarterly performance bonus: ~$2,200/quarter (? $8,800+/year average) offered after 1 year.

Medical Assistant
- Demonstrates ability to recognize urgent/emergent needs of patients and initiate appropriate emergency procedures as needed.
- Demonstrates current level of knowledge of various payor regulations by functioning within those guidelines.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Successful completion of a medical assistant program accredited by the Commission on Accreditation of Allied Health Programs (CAAHP) or Accrediting Bureau of Health Education Schools (ABHES).
- Certified Medical Assistant (CMA) certification or Registered Medical Assistant (RMA) certification must be obtained within first year of employment and maintained thereafter.
- Clinic/physician office experience preferred.
- Basic Life Support (BLS) for the Healthcare Provider certified or obtained within three (3) months of hire.
Proof of completion of Mandatory Reporter abuse training specific to population serve within three (3) months of hire

PRIMARY JOB DUTIES
1. Complete the initial, comprehensive, and ongoing assessment of patient and family/caregiver needs and provide direct or supervised nursing services based on a developed plan of care.
2. Develop an individualized plan of care, in collaboration with the hospice interdisciplinary team, patient, and family, based on assessment, identification of needs, and patient and family/caregiver goals and preferences. Incorporate palliative nursing interventions.
3. Communicate with the physician (Attending and Hospice) regarding the patient’s needs, response to treatment, and changes in the patient’s condition; obtain/receive physician orders as required.
4. Work closely with other members of the IDG, and in cooperation with family/caregiver to provide holistic physical, psychosocial, emotional, and spiritual services, including bereavement support to the patient and family to achieve the highest quality of care.
5. Demonstrates positive interpersonal relations in dealing with all members of the agency.
6. Effectively demonstrates the mission, vision and values of the Agency on a daily basis.
JOB SPECIFICATIONS
1. Education: Graduate of NLN accredited school of nursing and current license to practice professional nursing as a Registered nurse in the state; Bachelor’s Degree preferred.
2. Licensure / Certifications: Must have current Registered Nurse license in NC, NC drivers license with an automobile that is insured in accordance with state or organization, and a current American Heart Association CPR certification.
3. Experience: Minimum of one years’ clinical experience required; hospice experience preferred.
4. Essential Technical/Motor Skills: Working knowledge of Word, E-mail, typing of 30 wpm, basic technical or medical knowledge of home health billing requirements. Advanced customer service skills to respond with simple answers, etc.
5. Interpersonal Skills: Ability to work independently as well as effectively as part of an interdisciplinary team.
6. Essential Physical Requirements: Intermittent physical activity including walking, standing, sitting, lifting, and supporting patients
7. Essential Mental Abilities: Ability to concentrate with large volumes of paperwork and data entry, handle pressure of deadlines, good judgment, ability to organize and prioritize workload independently. Emotional/mental stability and stamina

The RN Educator / Infection Control Nurse takes on the following responsibilities for Eddy Memorial Geriatric Center (80 bed facility) and The Terrace Assisted Living facility (67 bed residence):
Design, develop and provide comprehensive nursing orientation to new-hires
Work closely with leadership and nursing staff to determine supplemental training opportunities, developing and providing new training on an as needed basis
Partner with the RN Educators across the Continuing Care Division to maintain a consistent and positive orientation experience
Monitor and support efforts to achieve and maintain positive quality measures
Oversee infection control efforts
Monitor infection prevention throughout the facility
Partner with the Infection Control Nurses across the Continuing Care Division to implement and educate policies and procedures at EMGC and The Terrace
Required:
A current license to practice as a Registered Nurse (RN) in the State of New York
Associate’s degree in Nursing or Degree of Nursing from an accredited school
3 years or more of experience as a RN preferably in skilled nursing
Experience as a RN in an educator or supervisory role
Preferred:

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
