Job Description

Under the general direction of nursing leadership and with provider collaboration and guidance, the Registered Nurse utilizes evidence-based practices in accordance with the State Nurse Practice Act and policies and procedures of Hartford Healthcare Hospital Outpatient Departments (HODs) and Medical Group, in a variety of clinical settings. This position facilitates care for a varied population of patients, which includes nursing standards of practice to meet the physical, emotional, spiritual, cultural, and educational needs of the patient and family.

  • Provides nursing care nursing care within a non-acute setting in collaboration with the inter-disciplinary team and provider according to the patients physical, emotional, spiritual, cultural, and educational needs of the patient and family.
  • Coordinates and provides holistic patient centered care.
  • Utilizes evidence-based practice in accordance with the department and/or medical group policies and procedures to ensure safe quality care and customer service.
  • Incorporates the art, advocacy, science and ethics in nursing effectively in their daily practice.
  • Demonstrates and actively supports research utilization through inquiring into evidence-based practices.
  • Evaluates and reviews the plan of care working with the interdisciplinary team to meet the diverse needs of the patient and family.
  • Identifies and reliably documents problems and expected outcomes based on a plan of care for a varied population of patients.
  • This role requires coverage at regional offices bringing variety to your workday and the chance to collaborate across multiple teams and communities.


Qualifications

Education

•        Graduate of an accredited School of Nursing.

The Assistant Nursing manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. ·        Interpret complex medical information to assess eligibility and recommend reasonable accommodations in compliance with ADA and related regulations.

·        Engage in the interactive process with employees, managers, HR partners, and medical providers to identify effective accommodations.

·        Act as a liaison between employees, healthcare providers, HR, and legal as necessary to ensure coordinated case management.

·        Document all case activity and maintain accurate, timely and confidential records in compliance with HIPAA and internal policies.

·        Collaborate with Leave Administration, Colleague Health, Occupational Health and Workers’ Compensation teams for integrated care and support.

·        Stay current on ADA, FMLA, and related federal/state legislation, and recommend policy updates as needed.



Qualifications

Education

·        Bachelor’s Degree in Nursing from an accredited school

Experience

·        5+ years of experience in clinical case management, Occupational Health, Employee Health or ADA Accommodations.

·        Working knowledge of ADA, FMLA, and related employment laws.

Licensure

·        Licensed RN in Connecticut
