As the Home Health Patient Care Manager, you are responsible for the overall supervision and coordination of clinical services. In the Patient Care Manager RN Hybrid role, you are responsible for the supervision and coordination of clinical services and provide and direct provisions of nursing care to patients in their homes as prescribed by the physician.
