- OBJECTIVE:
To provide Medical Clinical support services up to five (5) days per week. Provide family- orientated primary medical care services at Western Oregon Service Unit as needed. Within specialty area, assumes responsibility for the diagnosis, prevention, therapy, maintenance and rehabilitation of family members in the capacity of a family practice physician.

Qualifications
- MD/DO
- Degree:Doctor of Medicine, Doctor of Osteopathy from a school in the United States or Canada approved by a recognized accrediting body in the year of the applicant's graduation. A Doctor of Medicine or equivalent degree from a foreign medical school that provided education and medical knowledge substantially equivalent to accredited schools in the United States may be demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates (ECFMG) (or a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country).
- Graduate Training:Subsequent to obtaining a Doctor of Medicine, Doctor of Osteopathy degree, completion of internship, residency, and fellowship programs that are approved by accrediting bodies recognized within the United States or Canada.
- Board Certification: current board certification in Family Medicine or Med /Peds Dual certification
- Licensure: Candidates must have a permanent, full, and unrestricted license toto practice medicine in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. State licensure must permit independent practice. Provider must hold DEA controlled substances registration Certificate.
