This role collaborates with a multidisciplinary diabetes management team to educate patients and families living with diabetes and support individualized care plans.

Registered Nurse – Certified Diabetes Educator Responsibilities Deliver diabetes education to patients and families.

Utilize the nursing process and collaborate with interdisciplinary teams.

Administer medications and treatments according to policies and standards.

Develop and implement individualized patient care plans.

Document care in accordance with clinical standards (EPIC EMR).

Support performance improvement initiatives and shared governance.

Provide clinical expertise and guidance across departments.

Act as a patient advocate addressing clinical, psychosocial, and financial concerns.

Serve as a resource for staff regarding patient education materials and programs.

Registered Nurse – Certified Diabetes Educator Requirements Graduate of an accredited nursing program (BSN preferred).

Active RN license in Illinois and Wisconsin.

CDCES certification required.

BLS certification required.

Minimum 2 years of nursing experience.

CPR certification within 90 days if not already obtained.

Registered Nurse – Certified Diabetes Educator Additional Details Guaranteed 40 hours/week.

Overtime after 40 hrs at +$10/hr equivalent.

No on-call or holiday requirements.

Floating may be required based on competency and facility needs.

Facility size: 150–500 beds.

Registered Nurse – Certified Diabetes Educator Ideal Candidate Strong critical thinking and patient-safety focus.

Team-oriented with excellent communication skills.
