Job Description Saint Luke's Home Care & Hospice is seeking a Transitional Care Nurse to join our team Full-time Monday through Friday with flex scheduling to cover one weekend every 1-2 months for telemonitoring Bachelor of Science in Nursing required RN in MO & KS required Certification in Chronic Disease Management, Hospice and Palliative Care required (others will be taken under consideration) 2 years of experience required Clean driving record required Coverage area is Kansas City Metro, may assist in rural areas within Leavenworth, Jackson, Clay, and Platte counties Experience in Home Health, Hospice, Case Management preferred The Transitional Care Nurse TCN , under the direction of the Director of Operations, serves as a key point of contact for care coordination and transition management for Saint Luke's Home Care and Hospice patients who move from the acute hospital and facility to the home settings and within levels of care when already home.

The TCN will support patient self management, advocacy and oversee education and engagement, coaching and counseling of patient and family.

The Progressive Care Registered Nurse coordinates and delivers high quality, patient-centered care. In collaboration with medical providers, the RN provides pre-operative and post-operative nursing care. The RN serves as an advocate for patients to support an unparalleled patient experience.

What you will do in this role:

• Assess patient condition during admission and during each shift, identifying and reporting any changes in patient status.
• Perform procedures or other functions as ordered by the medical provider.
• Document the administration of care in the patient medical record in a timely and thorough manner.
• Perform the administration of prescribed medications and monitors response.
• Maintain a knowledge of medications, procedures, and equipment used in the care of cardiac and medical-surgical patients.

• Advanced Cardiac Life Support must be obtained within 60 Days of employment start date

We are seeking a Respiratory Student Extern for our team to ensure that we continue to provide all patients with high quality, efficient care. Did you get into our industry for these reasons? Under the general direction of the Director/Supervisor or designee, performs diagnostic and therapeutic procedures ordered by a physician. Duties include medical gas administration, aerosol therapy, obtaining and analyzing blood specimens, performing patient testing, operating and maintaining various types of respiratory and cardiopulmonary equipment, assisting with cardiopulmonary resuscitation, providing patient education, monitoring equipment and patient response to therapy. Additional responsibilities include completing patient documentation in the patient and departmental records, to include all computer based documentation. Cleaning equipment and maintaining safety and infection control practices. Accepting other job responsibilities as deemed reasonable by the Director/Supervisor or designee. In this position, the RCP will NOT be providing mechanical ventilation or maintaining artificial airways until they have successfully completed and graduated from their Respiratory Therapy program. The employee will provide care for birth to pediatric, pediatric/child patients between 1 to 11 years of age; adolescent patients between 12 to 17 years of age; adult patients between 18 to 64 years; and geriatric patients 65 years and older.
