Physician Assistant Role ScopeΒ 

  • Provides direct patient care in office consultations, including assessment, workups, triage, emergency work-ins, and other patient care activities. Position specializes in treating patients with kidney stones and other general urologic conditions
  • Obtains and documents health history, performs and documents physical exam, and orders and interprets diagnostic tests
  • Provides counseling to patients regarding diagnosis and expected course of treatment
  • Collaborates with physician(s) to provide appropriate direct care interventions within scope of practice
  • Collects, organizes, records, and discusses data as appropriate and uses sound clinical judgment based on best practice standards and evidenced based medicine
  • Prescribes pharmaceuticals according to patient needs and conducts appropriate patient follow-up & outreach
  • Ensures timely retrieval of laboratory and ancillary studies and diagnostic consultation reports
  • Consults with appropriate internal and external staff to make appropriate referrals for acute, chronic and preventive care
  • Coordinates patient care and visits with appropriate clinical and non-clinical staff

Qualifications

  • Master’s degree in nursing
  • National Certification in Advanced Practice and current CPR certification
  • Current State of CT APRN license with prescriptive authority and current DEA number
  • 2 years of experience (urology preferred)
  • Excellent clinical management skills and proven ability to work collaboratively in a team environment and take lead when appropriate

POSITION SUMMARY:

The Population Health Nurse is responsible for the review and implementation of clinical workflows and processes within the ECMP physician practices to ensure PCMHΒ level 3 recognition.Β  This role is responsible for the determination of a population of individuals whose disease acuity has been determined to be vulnerable, using a stratification process that incorporates data from available visits, claims, lab, medication, and admissions and discharges. Working closely with clinical support in the practices, staff, nursing and physicians, quality committee, NOC and IT, the Population Health Nurse is tasked with achieving system goals of improving clinical outcomeΒ for patients with chronic diseases by using timely and appropriate coordination of quality healthcare services to meet an individual’s specific health needs to promote positive outcomes.



Qualifications

EDUCATION/CERTIFICATION:

  • Graduate of an accredited school of Nursing.
  • Associate degree in an applicable field required /Β bachelor’s degree preferred. Β 
  • Must have a minimum of a current license as a Licensed Practical Nurse in the State of Connecticut.Β 

EXPERIENCE:

  • Must have at least five (5) years of nursing experience in a community practice health setting working with vulnerable populations.

COMPETENCIES:

  • Requires good analytical, organizational and interpersonal skills as well as the ability to communicate effectively in English, both verbally and in writing.Β Β 
  • Working knowledge of Microsoft officeΒ products.

  7. Provides ongoing support to ECMP practices to ensure that clinical standards are being adhered to.Β 
  8. In officeΒ contact is required from time to time to determine workflow changes. Support Practice Managers and clinical staff with workflow changes identified.Β 
  9. Determines ClinicalΒ quality initiative projects and implements them in ECMPΒ physician practices.Β Β Β 
  10. Participates in regular team meetings and peer review activities. Participates in departmental and organizational committees as applicable.
  11. Promotes collaborative teamwork; able to work with peers in a team situation.
  12. Maintain patient confidentiality and adhere to HIPPAΒ guidelines.

The Wound team LPN performs wound care in the daily delivery of services and maintenance of clinical care standards for clients receiving specialized wound care treatments. Works with the Wound Team RN, CWOCN and Manager of Specialty services to promote the daily operations and delivery of wound care to patients, and participates in the development, maintenance and implementation of program goals and objectives.

Participates in ensuring consistent delivery of quality care and communications; Performs wound care to promote cost effectiveness for wound treatments in relationship to patient needsΒ 

Seeks consultation with the Wound Team RN and WOCN staff for patient problems/issues that require a change in the plan/interventions designed to solve complex problems involving client care.

Participates in the interdisciplinary approach to client care with Wound program/Agency staff. The Wound Program LPN will perform both Wound care and medical/surgical services Care required by the patient to achieve quality outcomes. Informs the Wound Team RN, CWOCN and the physician/wound clinics of changes in client needs, wounds that require a change to the plan of care for wound care treatments and disease management

Assists with staffing needs to meet service demands.

Accountable for team performance in achieving desired clinical and operational performance measures.

Follows trends in specialty area of wounds for home care industry; makes recommendations of treatment options for incorporation into the program.

Participates in training, coaching of staff or onboarding to the Wound program

Participates and facilitates professional development needs and competency for wound program.

In addition, the position involves other duties such as:

1. Will actively participate in the performance improvement process known as H3W.

2. Participates in clinical education area to provide programs aimed to improve clinical competencies of direct service staff.

3. Research new information related to wound specialty and communicates to team.

Education
