Responsibilities
- 1. STANDARDS OF PRACTICE RN practice is guided by the ANA Scope and Standards of Practice (2015). The Standards of Practice describe a competent level of nursing care as demonstrated by utilizing the nursing process components of assessment, diagnosis, outcomes identification, planning, implementation and evaluation. Assessment: The registered nurse collects pertinent data and information relative to the healthcare consumer's health or situation Diagnosis: The RN analyzes the assessment data to determine the actual and potential diagnoses or the issues. Outcomes Identification: The RN identifies expected outcomes for a plan individualized to the healthcare consumer or the situation. The RN Case Manager - Home Health is responsible for the planning, coordination and delivery of quality home health care and services, ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Essential Activities and Tasks
Care Management and Service Delivery - 70%
Provides ongoing assessment and nursing care to patient in homes and health care facilities. Ensures an accurate evaluation of patient needs, develops and implements a comprehensive care plan that addresses current needs. For home health, identifies when patients need or qualify for additional services and notifies appropriate team member or team leader.
Follows current nursing professional standards of practice and agency approved protocols for home health.
Provides quality plan and symptom management for the comfort of patients.
Communicates with other health care providers about changes in condition and patient needs.
Assesses patient and caregiver needs and provides education when necessary.
Commitment to high quality customer service for internal and external customers.
Oversees the implementation of the patient’s plan of care. Evaluates the effectiveness of the plan on an ongoing basis.
Quality and Compliance Management - 30%
Audits and maintains current and accurate patient records through use of laptop computers and paper/chart documentation in accordance with policies, federal, state, and local guidelines.
Participates in quality improvement projects as needed.
Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.

Qualifications
Education
Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
Experience
One year experience in acute care or medical-surgical nursing required.
Experience in a Medicare-certified home health agency preferred.
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
Proficiency with electronic medical records (EMR) preferred.
Other Requirements

As a Transplant Registered Nurse, you will play a vital role in caring for pre- and post-transplant patients, including those undergoing kidney, liver, heart, and multi-organ transplantation. You will coordinate complex care plans, monitor for early signs of rejection or complications, administer specialized therapies, and provide essential education to patients and families navigating the transplant journey.

Responsibilities:
Patient Assessment & Monitoring
- Perform comprehensive assessments for pre- and post-transplant patients.
- Monitor vital signs, graft function, fluid/electrolyte balance, lab values, and indicators of organ rejection or infection.
- Recognize and respond to early symptoms of transplant complications, such as rejection, sepsis, or medication toxicity.
Transplant-Specific Care
- Administer immunosuppressive therapies, antirejection medications, and other transplant-specific treatments.
- Provide wound care, infection prevention, and management of surgical drains as applicable.
- Support coordination of biopsies, imaging, dialysis, and other procedures related to organ function.
Critical Interventions
- Respond promptly to emergent situations including acute rejection episodes, respiratory distress, or hemodynamic instability.
- Operate specialized equipment such as infusion pumps, telemetry monitors, and devices used in managing immunosuppressive drug therapy.
Collaboration & Communication
- Partner closely with transplant surgeons, nephrologists/hepatologists, pharmacists, social workers, dietitians, and transplant coordinators to ensure seamless care.
- Provide extensive teaching to patients and families regarding medications, lifestyle modifications, post-transplant surveillance, and long-term follow-up expectations.
- Participate in multidisciplinary rounds and contribute to individualized care planning.
