SCADDS Lebanon Pines program offers long term treatment for men with addiction and co-occurring mental health disorders on a 56-acre campus in southeastern region of Connecticut. ?The Pines? provides a structured recovery environment with a focus on psychosocial aspects of treatment. Residents work on recovery skills, managing the signs and symptoms of relapse and making meaningful lifestyle changes. Under the direction of our skilled and experienced staff, residents learn to recognize their disease, prevent relapse, and develop skills for reintegration into the community.

SCADD?s Transitional Housing is a residential treatment program with locations in Norwich and New London, Connecticut. The program is designed to assist and support men and women in their sobriety. This program affords individuals the opportunity to apply learned recovery skills, precent relapse and promote personal responsibility. Residents can integrate into educational, vocational, familial, and social structures.

SCADD?s Withdrawal Management program is a medically monitored withdrawal management and evaluation program delivered by medical and nursing professionals and enhanced by skilled clinicians and technicians. The services are delivered under a defined set of physicians monitored protocols. Twenty-four-hour nursing observation, medical monitoring and treatment are available on site. Counseling and case management services are available on site seven days per week.

Essential Duties and Responsibilities include but are not limited to the following.

  • Perform psychiatric assessments, formulate differential diagnoses, select and initiate appropriate treatment modalities, and order and monitor selected laboratory or routine diagnostic tests.
  • Prescribe psychopharmacological agents, administer psychotropic medications, and provide ongoing medication management and related treatments to assigned patients.
  • Work to ensure patient?s progress relative to established goals, reviewing and modifying treatment as necessary changes occur in conjunction with the patient and/or family.
  • Strive to provide patients with a full range of required treatment services including rehabilitative and support services with an emphasis on patient self-direction.
  • Collaborate with psychiatrist as needed to develop the most appropriate and effective psychotropic medication regimen according to the patient?s psychiatric history and active symptoms.
  • Collaborate with physician as needed to develop the most appropriate and effective Medication Assisted Treatment regimen according to the patient?s history and active symptoms.
  • Assist Clinic staff in coordinating activities of provider and nursing staff relative to medications and the provision of medical and treatment support services.
  • Collaborate as needed with staff from other clinical teams and other programs to facilitate continuity of care and appropriate services for eligible consumers.
  • Comply with practice standards, policies and procedures and all local, state, and federal laws governing patient care and the rendering of services.
  • Maintain responsibility for safe and effective patient care using evidence-based interventions and keeping current with new and proven treatment strategies.

MHS is hiring married couples to serve as Full-time Flex Houseparents. In this unique and life-changing career, couples reside in on-campus student homes and provide care, guidance, supervision, and support for a group of approximately 8–13 students living in the home. JOB SUMMARY:
Are you a compassionate and dedicated Licensed Practical Nurse (LPN) looking to make a real difference in the lives of others? RESPONSIBILITIES:
  • Provide direct nursing care to residents under the supervision of an RN or as directed by physician orders.
  • Administer medications and treatments (oral, topical, injections, other as permitted) per orders and facility policy.
  • Take and record residents' vital signs (blood pressure, temperature, pulse, respiration) and monitor for changes in condition.
  • Assist residents with activities of daily living (ADLs) as needed, including bathing, dressing, feeding, mobility, and hygiene.
  • Perform basic wound care, dressing changes, catheter or ostomy care (as allowed by facility policy).
  • Collect specimens for lab tests (e.g. urine, sputum) as ordered.
  • Observe and document residents' physical and emotional status and report any changes to the supervising RN or physician.
  • Maintain accurate and timely documentation of care provided, treatments administered, and resident observations in medical records.
  • Communicate with residents, families/POAs, and the healthcare team about resident status, care needs, and any concerns.
  • Adhere to infection control protocols, facility policies, and standards of care.


QUALIFICATIONS:
  • Valid, unrestricted LPN license in Connecticut.
  • Graduate of an accredited practical/vocational nursing program.
  • Current CPR/BLS (or equivalent) certification as required by facility policy.
  • Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.
  • Compassionate, resident-centered approach, with good organizational and time-management skills.
  • Comfortable working in a long-term care or skilled nursing environment (experience preferred).

RESPONSIBILITIES:
  • Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and personal hygiene.
  • Help residents with mobility: transferring, repositioning, ambulation, use of mobility aids as needed.
  • Take and record residents' vital signs (e.g. temperature, pulse, blood pressure, respiration) and monitor for changes in condition.
  • Serve meals and assist with feeding residents who need help; monitor and record intake and output as required.
  • Provide basic room care: change bed linens, maintain clean, safe, and sanitary resident rooms and common areas.
  • Assist with incontinence care, toileting, and maintaining resident dignity and comfort.
  • Observe residents' physical and emotional status; provide companionship and emotional support; alert supervising nurse to any changes or concerns.
  • Communicate with residents, families/POAs, and the care team about resident needs, status, and any observed changes.
  • Follow all facility policies including infection control, safety procedures, and resident confidentiality.

QUALIFICATIONS:
  • Valid Connecticut CNA certification.
  • Ability to read, write, speak, and understand English.
  • Compassionate, resident-centered attitude; strong interpersonal and communication skills.
  • Physical stamina and ability to lift, turn, move, and assist residents - including bending, standing, walking, and some heavy lifting - as required by resident care.
  • Comfortable working in a long-term care or skilled nursing environment; previous experience preferred but not required.
  • Dependable attendance and reliability; ability to work collaboratively as part of a care team while respecting resident dignity and safety.
