Job Description Summary:

Utilize evidence based nutrition care to assess and reassess patient nutrition risk and identify nutrition needs. Develop parental and enteral feeding regimes. Create and implement nutrition protocols. Participate in quality improvement initiatives/committees. Provide nutrition counseling to patients and families as warranted. Participate in multidisciplinary interaction and education.

Demonstrate proficiency in accordance with Standards of Practice and Professional Performance for Registered Dietitian Nutritionist while providing age-appropriate plan of care for patients and families.
Coordinates nutrition therapy with physicians and other health care professionals. Participates in multidisciplinary rounds and care planning sessions.
Plans, implements, and adapts nutritional care plan to meet physician orders, age, and disease specific physiologic needs. Intervention plan includes appropriate feeding route, mode, and nutrition prescription.
Monitors and evaluates patient response to medical nutrition therapy using specific goals and determines change if indicated.
Performs patient, family, and care giver education related to nutritional therapy and incorporates best method of learning style and communication into sessions.
Educates and trains interdisciplinary team members, students, and interns. Fosters teamwork and establishes effective strategies to implement innovative programs which leads to optimal clinical nutrition and financial outcomes.
Supports quality improvement, research, and outcome studies.  Identifies and communicates opportunities for waste reduction and service improvements.
Documents accurately and completely, including appropriate utilization of order writing privileges in electronic medical record.
Remains current on research developments and other methods of practice and incorporates this into current programs and services.
Supports the development and revision of nutrition related department tools, training materials, policies, and procedures.
Participates in the evaluation of clinical nutrition products and equipment.
Complies with all regulatory standards, including federal, state, and accrediting agencies and adhere to facility confidentiality, HIPAA regulations, and patient rights policies.
Participates in and attends departmental meetings, team development, and professional programs, as appropriate.

Job Description Summary:

OhioHealth Doctors Hospital is a progressive facility with the one of the largest DO Medical Training facilities in the area. The pharmacy practice model is mixed with a high degree of clinical services and desire for growth. This position leads the planning, organization, and management of the hospital's pharmacy services in collaboration with the Pharmacy Site Manager. He/She develops and implements appropriate pharmacy policies and practices. The scope of the position is focused on clinical pharmacy services, inclusive of the operations to support it, oversight of the infusion center and relationship with the oncology service line, interface with provider leadership, seated representative to system Pharmacy and Therapeutics Committee and leading local formulary efforts, engaged with the PGY1 Pharmacy Residency program, and matrixed relationship with system clinical services and quality. The position has approximately 30 direct reports (~22.5 FTEs) comprised of acute care clinical pharmacists, pharmacy interns, infusion center pharmacists and technicians, and medication reconciliation technicians.

Responsibilities And Duties:

Plans, organizes, leads, and manages all aspects of pharmacy services at both a care site as well as a service line level. Meets expectations for care site pharmacy services requirements by coordinating pharmacy activities between all care site stakeholders such as: medical staffs, nursing staffs, other departments, administration, etc.

Meets hospital and service line strategic goals by developing and revising departmental goals, policies and procedures as well as providing input in the development of system level policies and procedures. Assesses and executes on the implementation of system policies at the care site level. Coordinates activities between different pharmacy department areas, establishes and monitors services to meet customer needs i.e., patients, medical staffs, etc. . Serves as the primary liaison between departments at the care site level and communicates both care site specific and system wide information as appropriate to all teams that the manager is assigned membership.

Secures and retains staff of qualified personnel through recruitment, selection, training, and motivating. Addresses personnel issues to maintain a productive and pleasant work environment. Is primarily accountable for the ongoing evaluation and performance management of all associates with direct line of supervision. Engages the assistance and oversees the input from care site coordinators or team leads who may be contributors to these evaluations and performance improvement activities. Rounds routinely on all associates within the department and reports the results of that rounding to department and system level leadership. May be required to work in a staffing capacity should unusual or pressing circumstances dictate. Does have responsibility to participate in department call rotation.

In conjunction with Ohio Health compliance departments, assures pharmacy compliance with professional standards, TJC standards, state/federal agencies' rules/regulations and any other regulatory agency regulation by interpreting items and taking action to conform. Maintains responsibility to report up to both care site and system level leadership any/all compliance concerns and follow-up to any findings as may be needed. Works with nursing and medical leadership at the care site to coordinate shared compliance accountabilities when collaboration is needed to address or respond to findings.

Assists the department director in developing the department operating budget and monitoring expenses to assure pharmacy financial objectives are achieved. Works closely with the service line business director and care site procurement analysts/buyer on established purchasing and inventory control programs. Responsible for communicating up to care site leadership and system level leadership on the monthly fiscal and staff management reports.

Represents and integrates pharmacy services into hospital's total health care system by actively participating in appropriate committees as assigned at either the care site or the system level. - BS or Doctor of Pharmacy (PharmD) degree from an accredited institution (Required)

- Board Certification in Oncology (BCOP) or eligible for examination and attainment within the first 24 months of employment (Required)
- BCOP certification must be maintained during length of employment in position (Required)
- Completion of a Pharmacy Practice residency or equivalent hospital experience (Required)
- Pharmacokinetic drug dosing experience

JOB SUMMARY

The Oncology Clinical Pharmacist has a focused approach in working with OhioHealth’s multi-disciplinary oncology care teams. Primary responsibilities include:
• Promoting evidence and value-based oncology & supportive medication therapy through close participation in the oncology medication therapy decision-making process with the multi-disciplinary care team.
• Recommending modifications and improvements in oncology & supportive medication therapy based on information obtained through patient interviews, objective monitoring parameters and the patient medical record.
• Modifying chemotherapy plans in the electronic health record (EHR), in collaboration with the multi-disciplinary team, to ensure plans are accurate for patient’s age and performance status, pre-medications are appropriate for patient-specific factors and relevant drug interactions have been identified and addressed.
• Communication and documentation of findings and pharmacist interventions with providers and providing appropriate alternatives to current treatment plans as needed.
• Position may include optimization of patient medication adherence through counseling, education, and application of motivational interviewing skills. Documentation of patient education at therapy initiation and follow-up, including review of oncology & supportive medication effectiveness, side effects, lab monitoring, expected outcomes of therapy.
• Understanding of Specialty Pharmacy Benefit Managers (PBMs) and formulary requirements. Identifying and recommending alternative medications for requests for non-formulary specialty drugs required by insurances.
• Providing experiential instruction to Doctor of Pharmacy Students and OhioHealth pharmacy residents.

Responsibilities And Duties:

50% - Reviews physician orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies.  Contacts physicians to clarify orders as appropriate.  Dispenses and compounds medications and dispenses drug therapy-related devises in accordance to standards of practice and OhioHealth pharmacy policies.  Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity.  Ensures appropriate labeling for proper administration of medication. Interprets prescriptions and determines compatibility in accordance with standards of practice.  Prepares care plan for patient to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.
20% - Manages individual's drug therapy by initiating, monitoring and modifying prescription issued to patient in a consulting relationship with a physician.  Provides drug information to physicians, other health care professionals, and patients as needed.
10% - Counsels individuals with regard to their drug therapy by recommending drug therapy related devices and assisting in the selection of non-prescription drugs and appliances for the treatment of common diseases and injuries and providing instruction in their proper use.
10% - Assists with staff development and training.  Provides educational programs for pharmacy, nursing, and medical staff as needed.
10% - Directs and guides pharmacy technicians and pharmacy support personnel.
