POSITION SUMMARY:
Utilizing the nursing process, provides high quality direct and indirect patient –centered care. Coordinates patient care by working collaboratively with the interdisciplinary team. ESSENTIAL DUTIES and RESPONSIBILITIES:

· Performs population-specific comprehensive assessments and re-assessments of the patient's condition and care needs including the psychosocial, physical, educational and cultural aspects.
· Assess and reassess pain. Utilizes appropriate pain management tools and techniques. Educates the patient and family regarding pain management. Documentation reflects assessments, interventions and evaluation of effectiveness.
· Develops and revises individualized plans of care as indicated by assessment and patient response to treatment., collaborating with other disciplines as needed
· Follows the five (5) medication rights and reduces the potential for medication errors. Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition.
· Performs all aspects of patient care in an environment that optimizes patient safety and promotes positive patient outcomes.
· Documents the nursing process and responses to care in a timely manner utilizing the appropriate tools.
· Demonstrates an ability to be flexible, organized and function under stressful situations. Utilizes critical thinking skills and sound judgment in priority setting and delegation.
· Formulates and implements a teaching plan for patients and families based upon identified learning needs. Evaluates effectiveness of learning.

QualificationsEDUCATION/CERTIFICATION
• Must be a graduate of an accredited school of Nursing and have and maintain current a Registered Nurse license in the State of Connecticut.
• Must have certification in Basic Cardiac Life Support and Advanced Cardiac Life Support upon hire or receive certification within the new employee introductory orientation period and maintain clinical competency during employment in this role.
• Critical care experience preferred.

What You Need to Know:Be the Best Hospice Registered Nurse/RN Team Director You Can Be
If you meet these qualifications, we would love to meet you:
- Associate degree preferred
- Certified Hospice Nurse (CHPN) preferred
- Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)
- Minimum of one year of supervisory experience
- Possess and maintain valid CPR certification while employed in a clinical role (preferred)
- Must be a licensed driver who can travel to all business locations
- Meet the regulations and requirements of the state(s) in which the program provides services
Responsibilities:
As a Hospice Registered Nurse Team Director, you will:
- Supervise direct patient and family care team members, in collaboration with program site leadership
- Responsible for scheduling , payroll, and other administrative duties
- Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchonization
- Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdicisiplinary plan of care for each patient and family
- Collaborate with program site leadership memebrs to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
- Pariticpates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
- Collaborate with site leadership to support staff education efforts
- Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls
- Participates in or lead quality improvement activities, including data collection and aggregation as able
- Reinforces and supports all internal and external communications

POSITION SUMMARY:
Utilizing the nursing process, provides high quality direct and indirect patient –centered care. Coordinates patient care by working collaboratively with the interdisciplinary team. Communicates, as appropriate, the patient's clinical condition, responding quickly and accurately to changes in condition or response to treatment. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Be the Best Hospice Admissions RN You Can Be
If you meet these qualifications, we would love to meet you:
- Licensed as a Registered Nurse in the state(s) of practice, with an associate degree preferred and CPR certification recommended (required for direct patient care in Georgia). Must meet state-specific regulatory requirements.
- Previous nursing experience in hospice, home health, general medical-surgical, or population-specific care (e.g., pediatrics, geriatrics) is required, along with willingness to serve on-call.
- Must have a reliable vehicle, valid driver’s license, and the minimum required liability auto insurance.
- Strong communication and organizational skills, with working knowledge of Microsoft Office Suite and EMR systems. Must be able to work effectively as part of an interdisciplinary team.
- Physically able to lift patients over 200 pounds (with assistance), stand, walk, sit, stoop, and perform other physical movements frequently. Regular exposure to indoor environments and moderate noise expected.
Responsibilities:
- Conduct comprehensive assessments and collaborate with patients, families, physicians, and the interdisciplinary team to develop and adjust individualized plans of care
- Coordinate and communicate care needs with internal and external partners, ensuring timely updates, interventions, and resource utilization
- Provide patient and caregiver education, document care accurately, and maintain up-to-date clinical records that reflect current status and progress
- Participate in on-call rotations, admissions, and interdisciplinary meetings; manage care and scheduling using the Case Management model when serving as Care Manager
- Supervise and support other clinical staff as applicable; engage in ongoing education, special projects, and duties within scope of licensure
