The Opportunity
- Assist Medical Director in the realms of Quality and Patient Safety.
- Using Vituity resources, stay up to date with CMS' evolving quality metrics and requirements and learn how to reinforce compliance within Vituity and within your local practice operations.

Required Experience and Competencies
- Licensed physician as a Medical Doctor (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited medical school and completion of residency through an accredited residency program required.
- Maintain membership and privileges on Hospitalβs medical staff and comply with and abide by the bylaws, rules and regulations, and the policies and procedures of Hospitalβs medical staff where services are being provided required.

Assists in interdisciplinary classification of inmates by identifying behavioral, emotional, organic, drug/alcohol, or personality problems and the subsequent development of appropriate programs on an individual basis. Completes reports for institutional staff on inmates who may manifest sudden bizarre behavior or thoughts, insomnia, psychosomatic complaints, suicidal preoccupations, depression, somnambulism, anxiety, situational crisis, or long-term disciplinary and segregation cases, etc. Conducts individual treatment and crisis intervention sessions on an as needed or emergency basis. Responsible for providing assessment and referral services for staff referred to psychology services by their supervisors or for staff personally requesting assistance with problems of alcohol use, drug use, or emotional problems which are affecting job performance. Along with all other correctional institution employees, incumbent is charged with responsibility for maintaining security of the institution. The staffs' correctional responsibilities precede all others required by this position and are performed on a regular and recurring basis.

- Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
- Provide nursing care in the center, clinic, contracted facilities, and participantsβ homes according to each participantβs plan of care. (NJ: inΒ accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
- Participate in 24/7 βon-callβ process for triage of participants and their needs.
- Assess, plan, and coordinate participantsβ home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
- Monitor participantsβ acute and chronic care needs in all settings. Provide coordination and direct care as indicated toΒ promote continued care in the community or promote optimal institutionalΒ care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
- Ensure timely follow-up by providers on specialist visits and will assist withΒ obtaining specialist reports, facility documentation, and labs if needed.
- Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
- Notify participants of normal test results.
- Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
- Implement nursing-related care plan interventions.
- Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.Β
- Review and revises goalsΒ and approaches to participantsβ care in coordination with participant, family, caregiver and interdisciplinary team.
- Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.Β
- Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.Β
- Participate in all interdisciplinary team meetings.
- Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participantβs health problems.
- Supports OT as aback up to performing the duties of Home Care Coordinator on the IDT as needed/when assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed
- Actively participates in utilization review meetings and quality improvement projects / meetings.Β
- Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
- Participates in family meetings, staff meetings, in-service and training and orientation programs as required.

Position Summary
The Nurse Practitioner (NP) or Physician Assistant (PA) β Surgical First Assist is an integral member of the Saint Agnes Medical Providers (SAMP) Orthopedic Surgery team. This role supports a busy orthopedic practice and provides comprehensive patient care across inpatient, outpatient, office/clinic, and surgical settings. The NP/PA works collaboratively with the orthopedic surgeon(s) to ensure high-quality, coordinated care throughout all phases of treatment.
Responsibilities
- Provide high-quality, compassionate patient care in hospital in-patient units, outpatient clinics, office visits, and the operating room.
- Serve as a Surgical First Assist during operative procedures, adhering to best practices and safety standards.
- Perform patient evaluations, order and interpret diagnostic tests, and contribute to treatment planning.
- Conduct post-operative rounds, manage follow-up visits, and support continuity of care across care settings.
- Collaborate effectively with surgeons, nursing staff, therapists, and multidisciplinary teams.
- Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards.
- Educate patients and families regarding diagnoses, procedures, treatment plans, and recovery expectations.
Minimum Qualifications
- Graduation from an accredited Nurse Practitioner or Physician Assistant program.
- Current and unrestricted California NP or PA license.
- Current DEA registration and required certifications including BLS and ACLS.
- National certification as applicable to profession (AANP/ANCC or NCCPA).
- Previous surgical or orthopedic experience preferred but not required.
