ResponsibilitiesObtain necessary background information from assigned patient's chart.
Assist supervising Physical Therapist in obtaining specific evaluation data within areas of established expertise.
Seeks assistance from supervising Physical Therapist to obtain evaluation data outside of Physical Therapy Assistant experience.
Modify appropriate existing short-term goals within areas of treatment expertise for assigned patients and select and adapt treatment techniques to meet established goals and meet individual patient needs.
Discuss treatment plan with patient and family as indicated.
Complete all required clinical and billing documentation as indicated and submit in a timely manner.
Administer the prescribed physical therapy program to restore and maintain maximum function and prevent disability. The following modalities may be used for this purpose:
- Therapeutic exercises and activity for lower extremity function.
- Therapeutic exercises and activity for strengthening and balance.
- Exercises and activity to improve coordination.
- Instructions in self-care, including transfers, bed mobility and ambulation.
- Education in body mechanics, work simplification and joint protection.
Adhere to Symbria Rehab's mission, Code of Conduct, and policies and procedures.
Maintain productivity standards.
Full utilization of technology provided to document and enhance client care.
Adhere to terms and conditions of travel assignment contract.
Available and willing to travel to any location where Symbria provides contracted therapy services.
Must be able to provide own transportation between patient visits and travel assignments.

QualificationsTo perform this job successfully, the ability to perform each essential duty satisfactorily is necessary and the qualifications listed below are representative of the knowledge, skill, and/or ability required:
- Graduate of an accredited school for Physical Therapy Assistants.
- A valid registered license is required for Symbria's operating states, or the ability to obtain a license or compact license to support assignments in any designated state.
- If using a compact license, must have full license in your state of residence.
- Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
- Effective interpersonal skills with strong focus on customer service.
- Proficient use of computers and Microsoft Office: Word, Excel and Outlook.

TRAVEL ASSIGNMENT INFORMATION
- Assignments will be based on 13-week internal contracts. Once an assignment is complete there will be an option to extend the assignment or to be placed at another Symbria location. Terms of the contract are based on company and staffing needs.
- Each assignment is eligible for a monthly living stipend based on location and travel.
- Continuous assignments are guaranteed for one year. After completing a year of travel assignments, you may have the opportunity for relocation assistance to secure a permanent placement or choose to continue as part of the internal travel team.

We're seeking an experienced Injury Intake Paralegal—a uniquely skilled professional who understands the nuances of personal injury work and enjoys helping clients at the most critical moment: the very beginning of their case.
Unlike traditional paralegal roles focused on case administration, this position sits at the front end of the client journey. Coordination of Health Needs
- Identify on-going needs and services of residents in promotion of the highest quality resident services to be delivered.
- Ensure proper follow through and participate with evaluation and documentation for residents with a change in health care services and status.
- Assist Resident Care Director (RCD) in conducting resident evaluations and data gathers information at time of move in, 30 days post move in, semi-annually, and with any change in status.
- Conduct monthly wellness visits for all residents.
- Monitor and follow up with residents who have experienced any fall, accident, or incident and document and report as required and directed.
- Document all pertinent information in the resident wellness file.
- Maintain communications with resident's family and or responsible party to ensure medical needs for the resident are being met.
- Contact resident's attending physician when necessary and/or upon family request.
- Ensure weights and vital signs are obtained monthly for each resident.
- Assist in maintaining wellness files according to Sunrise policies and federal and state/provincial regulations.
- Maintain medical supplies and emergency kits for the community.
- Provide clinical support and assistance to community team members as needed.
- Understand and follow infection control practices.
- Partner with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks.
- Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions.
Medication Programs
- Monitor each resident's medication and treatment profile regularly to ensure each medication and treatment is administered as ordered and documented accurately.
- Transcribe physician orders Medication Administration Record Treatment Administration Record (MAR/TAR) to include initials, date and time 'noted', and date faxed to pharmacy accurately and promptly.
- Complete MAR/TAR audit each month.
- Demonstrate and is knowledgeable of the Six (6) Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
- Act as liaison for pharmacy services to ensure effective services for residents.
- Monitor the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
- Complete Medication Observations and skills checks for medication care managers at the direction of the RCD.
Quality Assurance and Regulatory Compliance

Experience:
• 24 months of role experience is required with some in the last 12 months.
• Must have at least 24 months of Medical Surgical - Orthopedic Unit experience.
• Experience with Epic is preferred from any number of months.
