Job Description 

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. This position reports to a Practice Manager. 

•        Provides office-based nursing care in collaboration with provider 

•        Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention 

•        Demonstrates independent nursing actions based on assessment and problem identification 

•        Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner 

•        Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy 

Travel to other regional offices, as needed 



Qualifications

  • Current state of Connecticut RN license and current CPR certification

POSITION SUMMARY:

The Cardiac Rehab RN supervises medically prescribed exercise therapy appropriate to the patient’s age, medical and emotional/mental status. The RN provides education classes, concentrating on client’s understanding of the cardiovascular system and heart-healthy lifestyle modifications necessary to maintain optimum health. The RN is responsible for orientation of cardiac and pulmonary patients and participating in Quality Improvement and KPIs. The RN will participate in the operational duties of the department, such as but not limited to:  Policy and Procedure maintenance, revising and development of billing, gathering of statistical data, developing departmental educational opportunities and participating in the on-going growth and maintenance of the excellent customer services provided to all that ECHN serves.

  • Participates in department QI and KPIs, data collection and reporting and Policy and Procedure maintenance.

  • Plans nursing actions to ensure individualized patient-centered care by integrating the patient, family, and/or multidisciplinary staff assessments in order to develop a comprehensive plan of care.

  • Conducts education classes for the patient and significant others as per education calendar according to appropriate teaching modules and documents in the patient’s chart.

  • Accurately completes necessary documentation including running daily and multi-session reports according to department policy.

  • Assesses the patient’s response to exercise and modifies the exercise prescription accordingly as per MD order and department policy.

  • Supervises exercise sessions for the patient, monitoring patient’s cardiovascular responses to exercise through blood pressure, telemetry, and RPE scale.

  • Identifies cardiac rhythms accurately and initiates appropriate interventions.

  • Develop a plan of care, formulating short and long term goals with the patient, document the progress of the         patient toward the expected outcome.

  • Consistently anticipates emergency situations and takes action to prevent or minimize a negative outcome by         demonstrating the ability to organize and delegate appropriately in emergency situations.

  • Collaborate cross site for the AACVPR credentialing process.

The Medical Imaging (Radiology) RN Clinician will be responsible for representing ECHN to the Radiology Intervention patient in both the outpatient and inpatient setting.  The Radiology nurse will report directly to the Manager of Vascular and Cardiology Services and indirectly to the Team Leader of Interventional Radiology. The Radiology RN Clinician provides nursing care by assessing the patient pre-intra and-post- procedure and assisting the Radiologist in the various diagnostic radiology testing. Examples are:  Interventional angiography, US guided thorocentesis / paracentesis, CT guided biopsies and any pt care needs that come up in the Medical Imaging Department.  The Radiology, RN Clinician, will administer moderate sedation and other prescribed medication or pharmacological agents pre,intra and-post- radiology procedure/imaging test.

Participates in the operational duties of the department, such as but not limited to; Quality Improvement, Policy and Procedure maintenance, revising and development of billing and gathering of statistical data, developing departmental educational opportunities and participating in the on-going growth and maintenance of the excellent customer services provided to all that ECHN serves as well as assisting the Manager in maximizing the department’s financial resources.

The Radiology RN Clinician also provides educational resources for patients and assesses each patient for understanding of procedures, post procedural education and lifestyle modifications necessary for maintaining optimum health and assists the department in planning and implementing new programs within the department.

 The Radiology RN Clinician accepts on-call responsibilities for the Medical Imaging Department.

·         Must be proficient in cardiac monitoring, administration of moderate sedation and be IV competent.

·         Must have strong verbal and written English communication skills in order to interface with staff and instruct patients.

JOB SUMMARY
