This job is intended for a physician to work with the Elder Care team in caring for Kaiser Permanente (KP) Georgia members 65 and older and those with Medicare needing special expertise of the team; the primary focus is on outpatient care, providing assessments, treatment, and recommendations for members with conditions common in older patients such as memory loss and falls. The physician will work closely with Geriatric RN Care Coordinator, Geriatrics and Palliative Care Clinical Pharmacist, and Geriatric Social Worker. Visits with patients will primarily occur in KP medical offices as face to face, virtual or telephonic visits. Complementing the Primary Care Physician (PCP), the physician in this position will treat and/or recommend care for patients' medical problems and chronic illnesses with emphasis on patients’ quality of life and functional ability, including addressing home safety and counseling on end-of-life care and other topics as appropriate. This position is par-time, 20 hours/week and based at our Kaiser Permanente Sandy Springs Medical Office.
Major Responsibilities/Essential Functions:
- Conducts telephone, virtual and office visits
- Assesses, treats, and/or makes recommendations for care for patients referred to Geriatrics
- Discuss and document goals of care discussion and advance directives as needed
- Addresses clinical issues and develops medical aspects of care plan (included but not limited to recommendations regarding observations, lab tests, medications, referrals, ordering tests and prescribing medications as appropriate) to be shared with and further implemented by the patient’s PCP, and others involved in patient’s care to optimize the patient’s overall health and functional status
- Respond to MD Advice and Referrals in the in-basket in a timely manner and provide appropriate clinical feedback to the referring provider
- Respond to e-consults from the PEEC clinic in a timely manner with recommendations appropriate to the patients’ pre-surgical needs
- Collaborate with other specialists such as Palliative Care, Neurology, Neuropsychology, Behavioral Health, and Clinical Pharmacy to provide comprehensive care when needed
- Supervise Geriatric NP, review and sign off on notes, provide feedback on care plan as needed, sign off on prescribed medications and other orders
- Provide education to Primary Care and other specialist regarding the care of the Geriatric patient as requested
Minimum Education and Experience:
- MD/DO, post-graduate residency training, and board certification in Internal Medicine or Family Medicine
- 1-year post-residency experience in caring for older patients in the outpatient and/or facility setting
- Demonstrated ability to effectively communicate with older patients, their family members/caregivers, and non-physician health professionals

JOB SUMMARY:
This job is for the care of Kaiser Permanente (KP) members in the Skilled Nursing Facility (SNF) setting. The provider will manage the care of the member, collaborate with other KP colleagues on the SNF rounding team, and interact with the SNF facility staff. The provider will manage members across facilities with the assistance of an NP and Acute Transitional Case Manager. They will escalate facility issues and patient care problems to the facility staff and appropriate KP staff in a timely manner. The providers' job responsibilities will be managed on-site in the facility.
- Round on Kaiser Permanente (KP) members at assigned contracted skilled nursing facilities (SNF) at least twice-weekly to address medical needs and concerns, as well as help with care planning going forward.
- Lead a team in SNF including a NP and KP Acute Transitions Case Manager to assure that the appropriate level of care is being provided and progress is being made towards goals in a timely manner.
- Discuss goals of care, transition planning, and advanced directives/POLST, in collaboration with the SNF facility staff and the KP Acute Transitions Case Manager
- Attend meetings with SNF staff, NP and KP Acute Transitions Case Manager to assist with discussions of ongoing care planning, discharge planning and coordination of care for members.
- Collaborate with evaluating KP specialists and other outside specialists as needed to drive care forwards while the member is in the SNF.
- Work with KP Acute Transitions Case Manager and transitions team to facilitate timely post-discharge follow-up for members after discharge from SNF.
- Work with KP Quality Resource Management Department, Care Without Delay and other teams as needed to accomplish regional goals.
- Establish and maintain good working relationships with other KP providers, on-site KP Acute Transitions Case Manager, contracted SNF medical and administrative staff, and transitions team.
- Assess alignment of patient care goals with care provided by SNF staff, identify systems barriers, and escalate issues when appropriate.
- Anticipate and plan for needs between visits, including potential discharge planning changes.
- Timely documentation of progress notes, discharge summaries, medicine reconciliation, etc. in KP HealthConnect
- Participate in departmental meetings.
- Obtain and maintain privileges at all assigned skilled nursing facilities.
- Take night call rotation during the week and throughout the weekend. Ability to work weekends if necessary.
- Must be able to drive distances between facilities to make rounds, lift 10 lbs. or more, alternatively sit and stand throughout the day, work on-site in a SNF.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
- Licensed physician in the State of Georgia.
- Board Eligible “New Graduates” in credentialed specialty when hired must become Board Certified within four years of the date of the earliest eligibility for the appropriate board to continue employment with TSPMG. Clinicians that are hospital based must follow the hospital policy for board certification requirements.

Emory Healthcare | Atlanta, GeorgiaEmory Healthcare, in collaboration with the Emory University School of Medicine, is seeking a Board Certified/Board Eligible Otolaryngologist to join our growing Department of Otolaryngology in a unique hybrid clinical role combining inpatient ENT hospitalist care with outpatient general otolaryngology practice.
The Department of Otolaryngology is nationally recognized for excellence in clinical care, research, and education, and offers a collaborative environment for physicians interested in both clinical practice and academic advancement.
This full-time position offers the opportunity to build a balanced clinical practice within one of the nation’s leading academic health systems while participating in teaching, research, and multidisciplinary patient care. The role includes inpatient care at Emory University Hospital and outpatient clinical practice at Emory Dunwoody Medical Center.

Clinical Responsibilities
Inpatient ENT Hospitalist Care:
· Provide consultative and procedural care at Emory University Hospital, a flagship tertiary care facility
· Management of common ENT consults; airway eval and management, vocal cord injections, bedside procedures
· Supported by experienced PA's who assist with consults, care coordination, and inpatient management.
· Multidisciplinary collaboration for care of complex patients including immunocompromised transplant populations
Outpatient Clinical Practice:
· Newly established Emory at Dunwoody, a modern community-based clinic offering comprehensive ENT services
· General ENT care for all ages with office-based procedures, evaluation and management of B&B conditions
· The clinic features audiology services with dedicated audiology booth, ultrasound capabilities
Schedule & Work Structure
· The hybrid schedule is flexible based on physician interests and experience, with a typical structure of 3–4 days per week inpatient hospitalist care and 2–3 days per week outpatient clinic
· Day shift hospitalist coverage (7 AM – 5 PM) with APP’s managing first-call and after-hours cases handled by the on-call team, allowing for a predictable daytime schedule.
· Call: Outpatient call averages 5-6 shifts per month

- Provide inpatient consultation services in palliative care
- Serve as attending physician and medical director for patients receiving inpatient hospice
- Facilitate clarification of patient and family goals of care.
- Consult with the attending physician and the Interdisciplinary Team to establish a written plan of care at intervals specified in the plan.
- Facilitate access to appropriate supportive care services.
- Education of students, faculty, staff and coworkers.
- Full and appropriate documentation of patient care services to support professional billing for these services.
- Participation in on-call coverage for hospice and palliative care patients and coordination of follow-up with the interdisciplinary team.
- Weekly participation in interdisciplinary team meetings to review active cases.
- Assist in the development of standard operating practices.

- Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.
- Teach, supervise, and evaluate students and house staff on hospice and palliative care clinical rotations in accordance with policies set forth by the educational leadership.
