The Registered Nurse ACT provides supportive services—including outreach, monitoring, assessment and service planning, support network building, consultation to families, side-by-side skills training and instruction in the use of community resources, including referrals to community assistance, as needed.

The Registered Nurse ACT delivers nursing care to individuals—in their homes or in the community—to assist with symptom management and to coordinate medical and psychiatric care.

JOB DUTIES AND RESPONSIBILITIES: Meet with ACT participants—in their homes and other community venues—to provide in vivo services which improve the participants’ quality of life and maintain community tenure.

Services include, but are not limited to: (a) Assisting with activities of daily living, (b) Budget planning and money management, (c) Maintenance of stable housing, (d) Accessing benefits and entitlements to increase financial independence, (e) Meal planning and grocery shopping, (f) Access to reliable transportation services, (g) Side-by-side coaching and training, (h) Transport and accompany clients to scheduled community activities or appointments.

Provide on-call crisis services on a rotating schedule (including both primary on-call responsibilities and back-up on-call responsibilities)—as directed by the Team Leader—to cover program needs.

Complete documentation of all services in a timely manner, using the electronic health record, in accordance with program and Network standards.

Work weekend, evening, and holidays on a rotating schedule—as directed by the Team Leader—to cover program needs.

Cross-train with other ACT Specialties to provide other services (e.g., housing support, vocational support, etc.) to the ACT participants.

Under the direction of the ACT Team Psychiatrist, the Clinical Coordinator, ACT Team Nurses, and the ACT Team Leader: (a) Packages medications for delivery to clients, (b) Provides medical support to ACT Team participants—e.g., attending medical appointments with participants, (c) Coordinates with medical providers and the ACT team staff, to ensure that health-care needs are met in a timely manner, (d) Complete health assessments at time of admission, in line with ACT criteria, for all participants, Maintain annual medical assessment reviews for all participants, (e) Maintain accurate documentation of participant medications in the Medication Administration Record (MAR), (f) Evaluation and documentation of medication effectiveness, complications, and side effects, (g) Arranging required lab work according to Network protocol.

The Registered Nurse ACT may provide educational groups to participants of the ACT Team.

These groups will address a variety of topics—including, but not limited to health-related topics, exercise routines, nutrition, medication management, etc.

The Registered Nurse ACT provides primary support to ACT participants and provides cross-training to the other members of the ACT team—to assist with medical wellness and healthcare—with the goal of promoting stability and increasing overall wellness.

PHYSICAL AND SENSORY REQUIREMENTS: Physical Demands: Heavy work exertional activity.

Occasionally lift and/or carry up to 50 lbs., and sit, stand, and/or walk at least 6 hours in an 8-hour workday.

Non-exertional activity: Stooping (bending at the waist) and crouching (stooping and bending at the knees)- required frequently; handling (grasping)- required frequently; fine manipulation- required occasionally.

Pay Rate: $27.14
- $36.29 / hour Duties: Analyze condition, troubleshoot failures, perform equipment repairs using standard procedures.

Rebuilding of mechanical equipment such as conveyor systems, gear boxes, vibratory units, and vacuum systems Perform problem solving and resolution of machine issues or reliability improvements.

Maintain machine standards of operation in accordance with baseline hardware and software installation / measurements / equipment standard setup manual.

Execute corrective, predictive, and preventative maintenance procedures to ensure a high level of machine utilization (OEE) and low scrap level.

Perform the equipment setup and change-over process.

Assist with continuous improvement of equipment / process safety, operation, and efficiency.

Support new system installations, R&D and engineering projects.

Train and mentor Technician I and Operators on team and across teams.

Maintain cleanliness and organization of the Maintenance shop.

Ensure the health and safety of personnel and care of the property, equipment, and product under their control.

Assist with Technician III duties when required.

Partner with other technicians on shift and cross shifts to continually complete objectives.

Perform other duties and tasks as assigned.

Position Requirements: Knowledge/Experience Knowledge in mechanical & electrical (motor circuits, VFDs, Servos, PLCs, single / three phase electrical systems; electrical voltages around 110v and above, up to 480v and electronics including 48v and below) Troubleshooting and repair of automated equipment.

Experience in reading, understanding, and interpreting blueprints, schematics and equipment documentation.

Knowledge of hand and power tool application, operation and use.

Knowledge of the high speed packaging line, roasting and grinding processing equipment.

(Preferred) Experience in machining, welding & fabrication.

(Preferred) Requirements: Background Check, Drug Test, Must be at least 18 years old.Able to Lift 50 pounds., required education: Associates Degree.
