Position Summary:

The Registered Nurse (RN) - Home Health is responsible for providing skilled nursing care to patients in the comfort of their homes. The RN - Home Health works in collaboration with the interdisciplinary team, physician, and family to develop a comprehensive plan of care to meet the patient's needs. As a full-time position, the RN - Home Health will be called upon to work on an as-needed basis, based on the needs of the organization. This is a full-time position.

Essential Duties and Responsibilities :

  • Ensure patient safety and provide high-quality nursing care
  • Assess the patient's condition and implement the care plan in collaboration with the interdisciplinary team
  • Provide wound care, medication management, IV therapy, and other skilled nursing services
  • Educate the patient and caregiver on disease management, prevention of complications, and self-care
  • Collaborate with physicians and other healthcare professionals to develop and implement the plan of care
  • Complete documentation of patient care in an accurate and timely manner
  • Participate in quality assurance and performance improvement activities
  • Attend and participate in interdisciplinary team meetings, as needed

Qualifications :

  • Active Florida RN license
  • At least 1 year of nursing experience, with Home Health experience preferred
  • Ability to work independently, with minimal supervision
