The Registered Occupational Therapist (Pediatrics) provides high quality assessment and treatment to pediatric patients to help develop, recover, and improve the skills needed for daily living and to help develop cognitive skills.


Essential Functions of the Job: [Other duties may be assigned.]

  • Prepare for, administer, score, and interpret initial evaluations and re-evaluations including writing a report.
  • Prepare for and provide therapy for treatment sessions.
  • Complete a daily note for every treatment session (should be completed within 72 hours).
  • Fulfill paperwork requirements for progress notes, plan of care updates, and prior authorizations based on insurance company.
  • Communicate effectively with parents and caregivers about treatment and home programming.
  • Develop home programming when appropriate.
  • Participate in staff meetings and/or staff coordination when appropriate.
  • Complete required tasks for billing.
  • Supervise COTA when applicable (including duties required to meet supervision requirements per state and licensure).
  • Provide assessment and recommendations for appropriate treatments
  • Treat various mental, emotional, and behavioral disorders
  • Appropriate use of PPE/infection control protocols when providing treatment to patients.
  • Plans and administers therapeutic treatment by telephonic/ telehealth methodologies as indicated by patient needs and state guidelines.
  • Review relevant literature, synthesize evidence-based materials
  • Conduct case management and other administration tasks as needed
  • Work with internal and external providers on patient referrals
  • Refer patients to specialists as needed.
  • Collaborate with NorthLakes providers as well as external resources and providers in the best interest of the patients’ continuation of care.
  • Work collaboratively with NorthLakes support staff in the interest of care integration initiatives.


Qualifications and Education Requirements

Master’s Degree in Occupational Therapy required and current, valid, and unrestricted license to practice the profession.

Preferred Skills

To perform this job successfully, an individual should have knowledge of Electronic Health Record Database software and Word Processing software.

Responsibilities

The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability, and adequacy of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects. Qualifications

Student or Graduate of a nursing program.

 

All entry-level associate degree registered nurses with a RN start date of April 1, 2020 and after must provide documented evidence of program completion of a baccalaureate degree in nursing program within five years from the last day of the month of the RN start date.

Additional Qualifications:

One year of RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic is preferred.

ABOUT THIS OPPORTUNITY
We are seeking skilled Make-Ready Designers (primary focus) and Power Distribution Designers to support engineering programs for major electric and telecom utilities. Ideal candidates bring demonstrated experience in overhead utility pole design, NESC application, clearance assessment, pole loading analysis, and construction package development. Useful design platforms include DDS, Katapult, O-Calc, GIS, MicroStation, AutoCAD, or client-specific drafting systems.


In this role, you will review field/walkout data, evaluate existing attachment conditions, model pole loading, determine make-ready requirements, and prepare accurate construction-ready deliverables. Job Duties

The RN Intake Coordinator screens potential new enrollees and evaluates and coordinates new enrollees to the appropriate Case Management Program. Assists Case Team Staff with outreach activities to support care initiatives for members throughout the care continuum.

This position is full-time, 40 hours weekly, 4 days per week - giving you more work-life balance and a weekday off!

The schedule is days, (4) 10-hour shifts per week, includes every other weekend, and every other holiday rotation. This role is office based and located out of Geisinger's Hughes Center in Danville, PA.

At least one year of RN work experience is required. Previous RN Case Manager experience is preferred

- Works with leadership to create appropriate support materials regarding program purpose, goals and objectives and deliverables.
- Meets with physicians across both Geisinger and the community to share details on the program.
- Identifies patients for recruitment and reviews with the appropriate primary and specialty care providers.
- Completes patient outreach and formal evaluation of all potential enrollees for the program to determine participant needs and eligibility for enrollment.
- Completes thorough review of all medical records including laboratory and radiology reports.
- Discusses care needs with primary or specialty care providers as needed.
- Interviews patient and family for in-home care needs as well as other behavioral or social gaps.
- Visits patients and families in their homes, at the hospital or nursing home for recruitment into program.
- Segments patients to the appropriate care team including home based medical team, community palliative care team or mobile integrated health team.
- Provides formal handoff to the appropriate care team.
- Schedules first visit with care team.
- Establishes and maintains an effective working relationship with County Area Agency on Aging and County Assistance Office delegates to provide a streamlined enrollment application process for Community Resources.
- Communicates with referral sources including physician offices, hospital discharge planning, and other community agencies for follow up in completion of the intake record.
- Provides follow up communication and correspondence to complete the enrollment process.
- Supports triage phone service taking acute care calls from patients and families or other care providers.
- Effectively assess acute care need.
- Manages gaps if able immediately.
- Identifies resources needed to respond to acute care need.
- Activates the correct care team or 9 as indicated by acuity of acute care need.
- Participates in weekly IDT meetings being prepared to share acute care activities for appropriate patients.
- Identified patterns to help the care team in Plan of Care adjustments.
- Maintains documentation of enrollment process, including prospective participants within designed software programs as necessary to complete mandatory state statistical reporting functions.
- Assists Administrative staff with completion of reporting requirements.
