Job Summary


B7E and 7E Step Down is a fast-paced, highly collaborative teaching unit serving medical patients suffering from an acute illness or injury, or experiencing an exacerbation of a chronic condition affecting one or more body systems. The unit has a strong respiratory focus but treats the full range of medical diagnoses.  Step down serves critical care patients with additional interventions including, but not limited to, cardiac drips, insulin drips, Bipap, and chronic vents.  


Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. The registered nurse who through knowledge and ability uses the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional spiritual, cultural and educational needs of the patient and family.

Job Summary
The Cardiac Catheterization lab at Hartford Hospital is comprised of four suites, two monitoring rooms, two scrub areas, a supply room and a holding area. There are approximately 5,000 diagnostic cardiac caths performed per year and of these, 1,500 become interventions. Hartford Hospital’s operating room provides 24-hour backup in the event that open-heart surgery is needed.

Care of patients undergoing a variety of Catheterization and Interventional procedures with conscious sedation/analgesia administration. Familiarity with IABP, cardiac drugs, and emergency care. Provides holistic, quality and cost-effective nursing care for patients/families and/or others for a patient population within a unit, service or other primary area of practice with an awareness of the episode of care. Introducing this excellent opportunity to join the obstetric team at Hartford Hospital. Our Labor and Delivery unit is a high risk referral center for the area and performs approximately 4,000 deliveries per year. Our Obstetric Triage unit evaluates more than 3,000 patients per year. As the volume and acuity of our patients has grown, we are looking to bolster our inpatient obstetric service with 24/7 APP coverage.  

Strong clinical reasoning and excellent communication skills required. This role functions both independently and in collaboration with nurses, residents, and attending physicians.  This position will report to the OB/GYN Hospitalist Director. 

Advanced Practitioner Role Scope:     

  • Care for patients admitted or being triaged across our Obstetric service line. 

  • Help on Labor and Delivery with admissions, magnesium sulfate checks and labor checks when residents unavailable, during sign-out and periods of higher volume. 

  • Rounding and evaluating our antepartum patients as needed based on volume and resident coverage.  Evaluation may include concerns for labor, elevated BP’s, infection or medication orders. Help with biophysical profiles (BPP’s) for non-reactive NST’s and limited obstetric ultrasound  

  • Typical postpartum care includes rounding, discharge planning, coordination of inpatient consults, responding to elevated blood pressures, postpartum or postoperative concerns, medication reconciliation and placement of Nexplanon.  

  • If needed in triage you will evaluate pregnant patients for pre-term/term labor, rupture of membranes, bleeding, high blood pressure, pre-eclampsia, abdominal pain and a variety of other complaints. There is always an attending assigned to cover our service serving as backup to the residents and APP’s. 

 

Schedule:  3 – 13.3 hour shifts per week (day and night rotation); night/weekend shift differentials apply

POSITION SUMMARY:

The Cardiac Rehab RN supervises medically prescribed exercise therapy appropriate to the patient’s age, medical and emotional/mental status.   •  Maintain professional growth and development through seminars, self-study, workshops, and professional affiliations. Conducts in-service for Cardiac Rehab/Stress Lab staff annually.

  • Represents the Cardiac Rehabilitation Department to the community through community based programs thereby increasing awareness of the prevention and treatment of heart disease.

  • Assesses the patient’s condition and care needs by collecting accurate and thorough data on admission , explains the Cardiac or Pulmonary Rehab program in detail and performs six (6) minute endurance walk(s) with the patient.

  • Accurately identifies actual and potential nursing diagnoses and/or problems.

  • Assesses patient and/or family learning needs, including the ability and readiness to learn. Identifies and arranges appropriate resources to assist in the educational process.

  • Participates in department QI and KPIs, data collection and reporting and Policy and Procedure maintenance.

  • Plans nursing actions to ensure individualized patient-centered care by integrating the patient, family, and/or multidisciplinary staff assessments in order to develop a comprehensive plan of care.

  • Conducts education classes for the patient and significant others as per education calendar according to appropriate teaching modules and documents in the patient’s chart.

  • Accurately completes necessary documentation including running daily and multi-session reports according to department policy.

  • Assesses the patient’s response to exercise and modifies the exercise prescription accordingly as per MD order and department policy.

  • Supervises exercise sessions for the patient, monitoring patient’s cardiovascular responses to exercise through blood pressure, telemetry, and RPE scale.

  • Identifies cardiac rhythms accurately and initiates appropriate interventions.

  • Develop a plan of care, formulating short and long term goals with the patient, document the progress of the         patient toward the expected outcome.

  • Consistently anticipates emergency situations and takes action to prevent or minimize a negative outcome by         demonstrating the ability to organize and delegate appropriately in emergency situations.

  • Collaborate cross site for the AACVPR credentialing process.

Responsibilities:
• Completes Initial Assessment for patients referred to the outpatient cardiac
rehabilitation program
• Identifies indications for participation, contraindications
for participation and specific monitoring needs and risk factor modification
goals for each patient.
• Completes patient education/counseling and documents treatment plan, including measureable goals, tailored to the individualized goals/needs of each patient referred to the program.
• Evaluates, monitors and documents multiple patients response to exercise therapy prior to, during and after exercise therapy. This ongoing assessment includes blood pressure, heart rate, rate of perceived exertion, telemetry/EKG, and may also include oxygen saturation, daily weight, blood sugar and ventricular assist devices
• Reports any significant participant acute conditions (e.g: dysrhythmias, blood pressure, angina symptoms, weight gain) to the supervising physician immediately. Reports ongoing, abnormal trends to physician/provider as needed (ex: elevated blood pressure; abnormally low blood pressure, episodes of hypo/hyperglycemia) via event report.
• Initiates ACLS measures and/or emergency care according to hospital policy.
• Completes tasks related to patient recruitment, insurance pre-certification, registration, and records management (Scott Care, Epic) as needed.
• Keeps current in the field of cardiac rehabilitation through journals, in-services, mock codes, conferences and staff meetings

    • Current BLS/ACLS
    • BSN required; MSN preferred

The Assistant Nursing manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. Qualifications

  • Graduate of an accredited nursing school
  • BLS certification by the American Heart Association
