Job Summary:

 A member of Community Health and Engagement, a registered nurse responsible for the provision of an organized program directed toward community education and health improvement. Utilizes nursing skills to analyze, develop, and implement community education programs based on departmental priorities.  Assists with colleague education related to caring for people from different cultures and diversity issues.  Works in conjunction with other members of Community Health and Engagement.

Job responsibilities:

·        Aggregates data to support community education and preventative health programs

·        Respond to public requests for health education programs.  Works with physicians, clinical staff, and community partners to staff events. Supports interventions targeted toward population health needs and program enhancements

·        Works with Human Resources and others on diversity issues related to colleagues, patients and community.

·        Coordinates and oversees various screening programs throughout the Hartford Region

·        Provides consultation and education to selected patient populations to facilitate health promotion.

·        Serves as hospital liaison to community organizations related to health education and community needs.

·        Maintains awareness of resources and regulations at the local, state and federal level.

·        Works closely with the Regional leadership team to ensure that the Hartford Region provides healthcare and community education in concert with health needs assessment.

·        Demonstrates knowledge of care for required age-specific populations necessary to carry out safe and effective performance of duties.

·        Participate in hospital functions and events that support and health and engagement.  May be required to vary shifts to meet the needs of patients and programs.

·        Travel between various locations within the community to perform duties.

·        Perform other related duties as required.

The role requires timely completion of workflows in Homecare Homebase (HCHB) and flexibility to meet agency needs.

Collaboration with teams is key to identifying trends and developing action plans to address them. Participation in quality and performance improvement initiatives is expected, along with supporting the agency’s compliance with data submission requirements for regulatory and payor standards.

JOB RESPONSIBILITIES

Reviews OASIS assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability.

Guides staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost-effective corrections that support clinical and operational goals.

Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager. Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs.

Collaborates to identify documentation trends, supports improvement planning, participates in quality initiatives, and helps ensure compliance and accurate data submissions across all settings.

Degree from an accredited educational program leading to licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP). Required

Bachelor's degree in nursing, master's degree in PT, OT, SLP preferred

The Float Pool at Hartford Hospital, also known as the STAR Team, is comprised of experienced, highly skilled Nursing professionals. B7E and 7E Step Down is a fast-paced, highly collaborative teaching unit serving medical patients suffering from an acute illness or injury, or experiencing an exacerbation of a chronic condition affecting one or more body systems. The unit has a strong respiratory focus but treats the full range of medical diagnoses.  Step down serves critical care patients with additional interventions including, but not limited to, cardiac drips, insulin drips, Bipap, and chronic vents.

The Case Coordination Department at Hartford Hospital is responsible for the assessment of patient needs for the most appropriate level of care across the continuum. Position Summary:

The primary focus of this position is on facilitating patient throughput. As a Case Coordinator at Hartford Hospital, assigned to a select inpatient population, you will assess patients for appropriate level of care; coordinate their care and services across the care continuum; identify risk factors; facilitate involvement of support services; perform utilization management functions; develop and implement transition planning activities; utilize and document aspects of care in our case management specific computer applications; and collaborate with other health care team members on process and quality improvement initiatives. Responsible for monitoring and responding to the overall health needs of students in the Clinical Day School. Responsibilities include performing initial student assessments (safety status, physical and/or emotional complaints, hygiene, etc.), monitoring self-administration of medications (prescribed or otherwise), maintaining state-required student health assessment records, prioritizing daily telephone calls regarding student’s health issues and/or medications, and observing, documenting and reporting medication issues/concerns to the appropriate MD or prescribing doctor.  This position requires a holistic approach to adolescent health and a working knowledge of health issues (medical and psychiatric) pertaining to the adolescent population. 

• CT licensure as a Registered Nurse is required.
• Cardiopulmonary Resuscitation (CPR) Certification is required.
• Age-specific knowledge of population and level of care age-specific competences are required.
• Familiarity with abuse and neglect reporting and duty-to-warn mandates and knowledge of community-based resources is preferred, but not required.
• A minimum of two (2) years experience.

• A minimum of one (1) year experience in Child and Adolescent Behavioral Health.

   

• This position requires strong clinical skills including a demonstrated strong team orientation.
• Strong written as well as verbal communication using the English language is essential.
• Must be flexible in approach so that service-specific as well as overall system needs can be met.
• Must be able to function effectively and independently in a work environment where multitasking is necessary.
