Job Title: Physician Assistant - Gastro Clinic

Location(s): All around Gwinnett County, 6 different locations

Hours/Shift: 8-5, M-F

Pay: $120,000-$140,000/year

Key Responsibilities:

Gastroenterology PAs shoulder a range of responsibilities, including:

• Conducting patient assessments, medical histories and physical examinations
• Ordering and interpreting diagnostic tests and imaging
• Assisting in gastroenterological procedures
• Collaborating with gastroenterologists to develop treatment plans
• Providing patient education on GI conditions and treatment options
• Monitoring patient progress and adjusting treatment as needed
• Maintaining accurate patient records and ensuring compliance with healthcare regulations
• Able to see 20-24 patients everyday. 10-12 in the morning and 10-12 in the afternoon

Setting

• Gastro clinic with mostly a female case load
• First patient in at 830 and last out by 430
• Endoscopic Procedures (EGD, Colonoscopy, EUS)

The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on medicare to keep them happy, healthy, and out of the hospital. Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient?s specific situation. Nurse Practitioners provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan.

Nurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health.

Core Responsibilities:

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)
• Assisting the care team with phone triage and outreach.
• Educating patients on their health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.

This role reports to the Center Medical Director and works closely with operational leadership.

Required Qualifications:

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in at least one of the following specialties:
  • Family Nurse Practitioner
  • Adult-Gerontology Primary Care Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontological Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization

Preferred Qualifications:

• Experience in primary care: internal medicine, geriatrics, or family medicine
• Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
• Passion for teamwork and the opportunity to collaborate cross-functionally

The typical pay range for this role is:

Piedmont Ear, Nose, Throat, and Related Allergy, a leading otolaryngology practice and an affiliate of SENTA Partners, has been serving patients in the Atlanta area for over fifty years. Our physicians collectively have over 100 years of experience and have advanced training and certification in otolaryngologic allergy, otology, laryngology, sleep medicine, and facial plastic surgery.The Physician Assistant position is a comprehensive role which includes support in the areas of case management and patient communications. The Physician Assistant works under moderate supervision to provide high-quality patient care and create favorable impressions with patients. The Physician Assistant will primarily be responsible for evaluating patients seen for follow-up visits. Although this person will at times be working independently, consultation with clinic physicians should be sought in difficult cases.

Key Duties and Responsibilities:

• Take a brief history of the chief complaint and/or inquire about interval history since last appointment for follow-up patients.
• Perform a physical examination pertaining primarily to the respiratory tract and skin. This exam will include examination of the nose, ears, and throat. In addition, auscultation of the chest will be performed, and, in selected cases, examination of the skin will be undertaken.
• A clinical assessment including differential diagnosis when indicated will be provided.
• Recommendations will be made for each individual patient experience. In more difficult cases, immediate consultation either directly, when a physician is in the office, or by phone will be necessary.
• It is mandatory that consultation with clinic physicians be sought in difficult cases.
• The Physician Assistant will help in management and treatment of reactions which occurs secondary to immunotherapy. Should the reaction be of greater severity, it is essential that consultation with a clinic physician be obtained.
• The Physician Assistant with prescriptive authority will be able to prescribe antihistamines, decongestants, mucolytics, bronchodilators, inhaled steroids (for chest and nose), oral steroids, and antibiotics.
• The Physician Assistant will develop treatment plans specifically tailored to each individual patient, document the plan clearly in the chart and communicate in a clear and concise manner to the patient.
• The Physician Assistant will order specific laboratory assessments, radiology, and CT scans as deemed appropriate to the medical condition.
• The Physician Assistant should be able to take call from their home during after-office hours. During these times, physician back-up must always be available for consultation.
