Responsibilities:
As an APP at Duly, you will get the opportunity to:
- Practice at the top of your license and form trusting relationships working under the direct supervision of your collaborating physician, Dr. Ali Imtiaz, DO | Duly Health and Care.
- Gain increased autonomy and independence in decision making with support from Dr. Imtiaz.
- Impact care delivery and patient satisfaction by having a voice in clinical decisions.
- Develop a 1:1 relationship and partnership with Dr. Imtiaz clinical staff members, and management.
- Join a collaborative team and participate in a structured training program including orientation, shadowing, and onboarding
- Complete patient examinations and treatment, order diagnostic and therapeutic modalities, prescribe medication, provide patient and family education, and complete documentation.
- Strong internal referral network and collaborative provider partners across Nephrology and more than 1,600 Duly providers
- Live and work in the vibrant suburbs of Chicago with excellent schools and convenient access to some of the nation's leading healthcare facilities

Qualifications:
- Graduation from an approved school for Nurse Practitioners OR Physician Assistant-Masters Degree.
- Licensed or ability to obtain licensure in the state of Illinois
- Previous Experience (1+ years as practicing APP) in Nephrology or similar setting.
