As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.
As a Home Health LPN, you will:
- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes.
- Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.

Required Qualifications:
- Current nursing license in the practicing state
- Valid drivers license, auto insurance and reliable transportation

Pay Range
β’ $33.00 - $47.00 pay per visit/unit
β’ $53,000 - $73,000 per year base pay

Driving Statement:This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. JOB SUMMARY:
The Medical Assistant provides clinical care to patients in urgent care, occupational health, and workersβ compensation settings. Responsibilities include initial evaluations, x-ray testing, and preparing patients for provider evaluation by recording histories, vital signs, and documentation while recognizing acuity levels for appropriate intervention.
KEY RESPONSIBILITIES:

β’ Greet patients promptly and provide clear communication about next steps and wait times
β’ Offer solutions to patient concerns and seek help in emergent or hazardous situations

β’ Support front desk staff with check-in and check-out procedures when needed
β’ Accurately document vital signs, histories, medications, allergies, and other pertinent details
β’ Assist with patient transfers, procedures, and lab work under provider supervision
β’ Prepare and shoot X-Rays, ensuring proper positioning, safety, radiation protection, and shielding
β’ Complete all required visit documentation and maintain stocked, clean exam rooms
β’ Adhere to infection control, safety, and patient confidentiality standards
β’ Perform phlebotomy, injections, and other clinical procedures as trained and directed
