Will work effectively on a complex interprofessional team. Maintains ethical principles and professional standards of practice in adherence to NASW Code of Ethics. The social worker is a part of the health care team and establishes and maintains a relationship with community agencies, has a working knowledge of community and financial resources, and assists with appropriate agency referrals and short and long-term planning as needed.

Qualifications

Program Summary
In the Adult Partial Hospitalization Treatment and Intensive Outpatient Treatment programs, we focus on creating a therapeutic environment where people can learn strategies for coping and improve their functioning. Each client's treatment plan is centered on group therapy which allows for sharing of experiences while gaining support and validation. When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Natchaug brings this clinical care closer to people's homes by offering programs throughout Eastern Connecticut.

Job Summary

Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Adult and Child/Adolescent partial hospitalization program. Clinical services include individual, group and family therapy. Inter-agency collaboration with and exposure to school social workers, principals, DCF, and court systems.

Key responsibilities of Behavioral Health Clinician:

  • Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
  • Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning.
  • Refers patient to appropriate levels of care and available resources that promote the individualized care plan.
  • Maintains timely patient-centered documentation that reflect the patient and client systems' pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.

Schedule: Monday - Friday; 8:00 AM to 4:30 PM. This position floats to any Natchaug clinical site as needed; including, Vernon, Enfield, Dayville, Danielson, Mansfield, Norwich, Groton, and Old Saybrook. Additional compensation provided.

***Behavioral Health Clinicians are eligible for a Sign On Bonus up to $2,500.00, in accordance with program guidelines***

Qualifications

  • Master's degree in social work, counseling, or marriage and family therapy or related human service field.
  • Previous experience in a healthcare setting required, preferably working with a mental health and/or chemical dependency population.
  • Current Connecticut LMSW, LPC-A, LMFT-A.
  • Excellent communication skills, with the ability to establish relationships within and outside their entity
  • Is cognizant of health-related social needs and cultural competence. Recognizes and addresses health disparities-inequalities and injustices directed toward clients, related to access to care and provision of health services.
