Stroke Program Physician Assistant or Nurse Practitioner (APRN) Role Scope

·        Provides holistic, quality and cost-effective advanced practice care for patients, families, and others in a target population, and participates in the medical management of patients, to achieve or maintain optimal health status, manage disease and reduce risk of illness.

·        Engages in independent decision-making regarding the health care of individuals, families and groups across the lifespan and across the health care continuum.

·        Reports to Nursing Director, Medical Director or Nurse Manager, or a matrix which may include Nurse Manager, Nursing Director, Medical Director, or some combination of those.

·        The Physician Assistants and Nurse Practitioners (APRN) are part of a multidisciplinary team that plans and delivers care for neurology, stroke and other complex disorders affecting blood supply to the brain and spinal cord, including brain aneurysms, carotid artery disease, cervical artery dissection, vascular malformations and other disorders of the nervous system under the direction of the attending physicians at Hartford Hospital.

Job Summary:

The Registered Nurse in the Centralized Monitoring Unit (CMU) plays a key role in supporting the safe and efficient monitoring of nearly 300 patients’ cardiac rhythms and hemodynamic status. This is a non-direct care position, ideal for a clinically skilled RN with experience in telemetry who enjoys working in a fast-paced, technologically advanced environment.

The RN in this role provides clinical support to a team of Central Monitor Technicians (CMTs), promotes adherence to protocols, and facilitates clear communication with inpatient care teams. This position contributes to the overall quality of care by helping ensure accurate cardiac monitoring, appropriate escalations, and consistent workflow.

Job Responsibilities:

· Support and coordinate the daily activities of Central Monitor Technicians to ensure timely and accurate telemetry monitoring.

· Serve as a clinical resource for rhythm interpretation, protocol adherence, and telemetry best practices.

· Collaborate with the CMU Nurse Educator to help provide training and reinforce department policies and procedures.

· Support smooth transitions between shifts to maintain continuity of patient monitoring.

· Participate in daily team huddles and contribute to unit-based initiatives.

· Act as a communication link between the CMU and inpatient clinical staff, promoting timely updates and coordination.

· Assist in reviewing staffing assignments and making adjustments as needed.

· Contribute to quality improvement efforts related to telemetry and monitoring workflows.

· Assist with coaching and guiding staff in real-time situations, promoting best practices and a supportive team environment.

· Demonstrate commitment to Hartford HealthCare’s mission and values in daily practice.

BSN required; MSN preferred. One degree must be in Nursing.

· Candidates actively pursuing a BSN or MSN may be considered, with continued progress toward degree completion expected within five years.

Job Summary

B7E and 7E Step Down is a fast-paced, highly collaborative teaching unit serving medical patients suffering from an acute illness or injury, or experiencing an exacerbation of a chronic condition affecting one or more body systems. The unit has a strong respiratory focus but treats the full range of medical diagnoses.  Step down serves critical care patients with additional interventions including, but not limited to, cardiac drips, insulin drips, Bipap, and chronic vents.  

Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. Job Summary
The Cardiac Catheterization lab at Hartford Hospital is comprised of four suites, two monitoring rooms, two scrub areas, a supply room and a holding area. There are approximately 5,000 diagnostic cardiac caths performed per year and of these, 1,500 become interventions. Hartford Hospital’s operating room provides 24-hour backup in the event that open-heart surgery is needed.

Care of patients undergoing a variety of Catheterization and Interventional procedures with conscious sedation/analgesia administration. Familiarity with IABP, cardiac drugs, and emergency care. Provides holistic, quality and cost-effective nursing care for patients/families and/or others for a patient population within a unit, service or other primary area of practice with an awareness of the episode of care. Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. Registered Nurse (RN) will work in our outpatient Infusion Centers providing holistic, quality nursing care for patients/families and/or others for a patient population within the Infusion Centers. Responsibilities include but, are not limited to the following:

• Practices safe administration of chemotherapy/biotherapy, infusions, injections, and transfusions.

• Manage a caseload of patients receiving a variety of infusion and oral therapies on an outpatient basis.

•Participates and engages in continuous professional development, safety behaviors and focus on the patient experience.

Job Summary
Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. The registered nurse who through knowledge and ability uses the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional spiritual, cultural and educational needs of the patient and family.

·        Current Connecticut RN License.

·        Graduate of an accredited nursing school

·        ASN Required (Must obtain a BSN within 6 years of starting in the RN role)

·        BSN preferred.

·        BLS certification by the American Heart Association required prior to date of hire.
