Position Title: Care Manager (RN)
Work Location: Member-Facing - Region 8 - Charlotte, Collier, DeSoto, Glades, Hendry, Lee, and Sarasota
Assignment Duration: 3 Months with a possibility of extension or conversion
Work Schedule: M-F 8 AM-5 PM

Position Summary: This position will be with the Enhanced Care Coordination team for the Children's Medical Services plan within The Organization. This team serves medically fragile children aged through 21 years. Specifically, this team primarily serves members with private duty nursing services in the home.
Background & Context: While the team serves members throughout the entire state of FL, we are very close-knit and collaborative. We communicate with one another frequently, sharing resources and supporting one another daily. Leadership is highly engaged and approachable. We are a very positive, family-centered team. Key Responsibilities:
- Making phone calls to parents of members to collect information to complete annual assessments, monthly required update call, a monthly face-to-face visit and if visit declined a second monthly call or a virtual visit replaces it.
- Calling DME companies and providers if a parent states a problem.
- Managing difficult parents at times, responses to escalations and/or complaints as needed.
- Monthly regulatory reporting compiling updates obtained on each member of their caseload.
- Complete all required calls/contacts/visits or calls in place of visits, collect assessments on time, compile monthly information regarding member status into an assessment called an ECC note that is required by end of month.
- Teams meeting or a phone call with the PDN agency every 6 months to review the care plan agency has provided.
- Great documentation needed, education provided.

As a CT Technologist, responsibilities include but are not limited to:
- Select proper technical factors for study then perform and complete diagnostic CT examinations accurately in a caring, compassionate and respectful manner
- Possess sound and thorough operational knowledge of the CT scanner, pressure injector, resuscitation equipment, laser filmer, and teleradiology system
- Assist in the administration of contrast media and other related procedures
- Prepare, transfer and position patients as needed ensuring modesty, safely utilizing auxiliary equipment and/or devices indicated
- Document all pertinent clinical information prior to exam, obtain informed consent, and complete all required fields within MEDITECH accurately and on time
- Obtain high-quality CT images assuring only optimal images are submitted for interpretation
- Remain proficient in executing software protocols and keep up to date with changes in the field by in-service study, attending courses or seminars as necessary/recommended
- Maintain proactive and effective communication with patient and other caregiver colleagues to include the radiologist
- Check the operation of the equipment, maintain cleanliness of table/equipment to prevent cross-contamination of contagious diseases and keep room stocked with linens and necessary supplies
- Accept on-call and call-back responsibilities as needed to provide optimum patient care
Qualifications
- Completion of a 24-month program in Radiologic Technology at an accredited, AMA-approved institution
- ARRT (CT) certification (preferred)
- Clear/active Florida Department of Health licensure (CRT)
- Current Basic Life Support (BLS) certification
- Ability to perform work which requires frequent standing, bending, reaching, squatting, kneeling, moving, and/or lifting of patients/equipment up to 50 lbs.
- Minimum of 1-3 years current/recent experience in diagnostic radiology, preferably in an acute-care hospital setting
