At Park Manor Health & Rehabilitation, our nurses play a critical role in helping residents recover, regain strength, and return home safely. As a Day Shift LPN on our Rehabilitation Hall, youβll provide hands-on nursing care while working closely with RNs, therapists, and the interdisciplinary team to support each residentβs recovery journey.

β Position Overview: LPN β Rehabilitation Hall (Day Shift)
The Day Shift LPN on the Rehabilitation Hall provides direct nursing care to residents recovering from illness, injury, or surgery. Working under the direction of the RN, the LPN supports care coordination, medication administration, and clinical monitoring to ensure safe and effective rehabilitation outcomes.
Our 12-hour, 7-on/7-off schedule allows for excellent work-life balance while maintaining continuity of care for our residents.
Key Responsibilities
- Provide direct nursing care to rehabilitation residents
- Administer medications and treatments according to physician orders
- Monitor resident conditions and report changes to the RN or physician
- Assist with development and implementation of care plans
- Document resident care accurately and timely
- Support therapy and rehabilitation goals in coordination with the care team
- Supervise and assist CNAs with daily care activities
- Maintain infection control and regulatory compliance standards
Qualifications
- Active Licensed Practical Nurse (LPN) license in good standing
- Long-term care or rehabilitation experience preferred
- Strong clinical and documentation skills
- Compassionate, resident-centered mindset
- BLS/CPR certification required

The primary purpose of the Registered Nurse (RN) is to utilize the nursing process in planning and providing direct care to residents as assigned. The nurse will plan, direct and carry out shift routines in assigned nursing unit and observe, direct and evaluate the activities of personnel as the person in charge according to the Nurse Practice Act. Will assume Building Charge responsibilities as directed by the DON or Designee.
QUALIFICATIONS:
- Current licensure in the state of Colorado as a Registered Nurse.
- Current CPR Certification.

- Initiate disciplinary action when the performance of personnel warrants such action.
- Report occupational exposures to needlesticks, blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policy and procedures governing accidents and incidents.

- Maintains competence in all required skills within assigned area(s).
- Treats and ensures that residents are treated with respect and dignity.
- Immediately reports any suspected abuse or neglect per company policy

- Administers prescribed medications and treatments per MD orders.
- Is drug knowledgeable and reports resident response, side effects, responds to drug considerations and reports problems or potential problems to the physicians.
- Maintains all required documentation in an accurate and timely manner according to policies and procedures of the facility.
- Follows all shift routines, policy and procedures regarding monitoring of vital signs, weights, administration of medications and treatments.
- Make physician/provider calls to report resident change in condition.
- Keep Resident Representative and families updated on resident conditions.
- Comply with acute emergency protocols and tasks involved in the event of a 911 call.
- Follows all Advance Directives: DNR/DNI/CPR/Comfort Care protocols and orders.
- Charting, Documentation and Assessment Functions
- Contribute toward completing and updating resident's care plans.
- Evaluate and reports the performances of all personnel working under your supervision and direction.
- Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
- Accurately transcribes physician's/provider's orders as per facility protocol and forwards necessary information to other departments.
- Document and report all incidents, accidents that occur involving residents, visitors, and staff.
- Chart nurses' notes in an informative and descriptive manner that reflects the care provided as well as the resident response. Follows VOA Policy and Procedures.
- Provides ongoing resident assessments as necessary and as assigned and documents these assessments per facility policy. Utilizes E-Interact COC & Care Paths for change in condition.

- Maintain a clean, safe and efficient work environment.
- Maintains continuing education requirements.

- Graduate from an accredited school of nursing with current licensure as a Licensed Practical Nurse in the state of Colorado.

- Charting and Documentation Functions:
- Maintains all required documentation in an accurate and timely manner according to policies and procedures of the facility, including documentation of medications, treatments, restraints, psychotropic medications and skin condition.
- Contribute toward completing and updating resident's care plans, as directed by RN.
- Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
- Accurately transcribes physician's orders as per facility protocol and forwards necessary information to other departments.
- Document and report all events that occur involving residents, visitors, and staff.
- Chart nurses' notes in an informative and descriptive manner that reflects the care provided as well as the resident response. Follows VOA Policies and Procedures.
- Provides ongoing data collection and completion of forms as necessary and as assigned.
- Ability to maneuver and utilize the Electronic Health Record for documentation.
- Personnel Function
- Attend nurses meetings as scheduled.
- Maintain a clean, safe and efficient work environment.
- Maintains continuing education requirements.
- Participate in employee performance evaluations, provide input into your shifts staffing requirements, and making recommendations to the Designee RN, DON, ADON or Nurse Manager concerning employee counseling etc.

JOB SUMMARY:

RESPONSIBILITIES:
- Provide direct nursing care to residents under the supervision of an RN or as directed by physician orders.
- Administer medications and treatments (oral, topical, injections, other as permitted) per orders and facility policy.
- Take and record residents' vital signs (blood pressure, temperature, pulse, respiration) and monitor for changes in condition.
- Provide assistance with activities of daily living (ADLs) when needed, such as bathing, dressing, feeding, mobility, hygiene.
- Perform basic wound care, dressing changes, catheter or ostomy care (if within scope and facility policy).
- Collect specimens for lab tests (e.g. urine, sputum) as ordered.
- Observe and document residents' physical and emotional status; report any significant changes to supervising RN or physician.
- Maintain accurate and timely documentation of care provided, treatments administered, and resident observations in medical records.
- Communicate with residents, families/POAs, and the healthcare team about resident status, care needs, and any concerns.
QUALIFICATIONS:
- Valid, unrestricted LPN license in Connecticut.
- Graduate of an accredited practical/vocational nursing program.
- Current CPR/BLS (or equivalent) certification as required by facility policy.
- Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.
- Compassionate, resident-centered approach, with good organizational and time-management skills.
- Comfortable working in a long-term care or skilled nursing environment (experience preferred).
