Responsibilities

Our Physical Therapists examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team. Responsibilities

Our Occupational Therapists help patients who have debilitating conditions improve the functions of performing everyday tasks in the home. Occupational therapists use treatments to develop the daily living skills of their patients, the basic motor functions of patients as well as to compensate for any loss of function that may have occurred with the patient.

Qualifications

* Must be a graduate of an approved school of Occupational Therapy and licensed as an Occupational Therapist/OT in the state in which he/she currently practices.
* A minimum of one-year experience is preferred, but can be less with Regional Vice President and/or Regional Director of Therapy Services approval based on prior experience setting.
* Medicare home health experience is preferred.

Responsibilities

In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. Responsibilties:

Provide in-home, para-professional services necessary for caring for patients' personal needs under the direction of the RN.

Β 

  • Β Perform and/or assist patient with personal care activities, such as, baths, showers, skin and hair care, and oral hygiene.Β  Assure universal safety precautions and direction on the care plan are followed when providing care.Β  Provide care that meets the patient's needs.
  • Notify RN of any changes in patient condition observed in a timely manner and document appropriately.
  • Record all required information on patient chart completely and accurately.Β  Assure documentation is completed per organization protocol.
  • Assist patient with transfer and ambulating.Β  Perform range of motion exercises.Β  Assure all activities are performed safely and follow the principles of good body mechanics.
  • Perform general housekeeping of patient's area and change patient's bed linens.Β  Assist family with light housekeeping activities, such as dusting, vacuuming, damp mopping, etc.
  • Assist client with self-administration of oral medication/s.Β  Remind patient when to take medication/s.
  • Prepare simple meals in accordance with dietary instructions and assist patient with eating if necessary.
  • Assist with of foley catheter and colostomy care to ensure proper hygiene.Β  Assist patient with changing colostomy bags.Β  Perform peri care.Β  Measure urine.Β  Assist with ostomy care and assure skin is clean around stoma.Β 
  • Other duties and/or projects as assigned.Β  Demonstrate flexibility with job responsibilities in all areas.
  • Adheres to HMH's Organizational competencies and standards of behavior.
  • Β Lifts a minimum of 50 lbs., pushes and pulls a minimum of 50 lbs. and stands a minimum of 8 hours a day.

The Physical Therapist performs physical therapy evaluations and treatments as ordered by qualified physicians in accordance with company standards and federal, state, and local standards, guidelines, and regulations to ensure that the highest degree of quality care is maintained at all times.

Essential Activities and TasksΒ 

Expectation and Care Management - 60%

  • Initiates contact with physician for orders and treatment plan formulation and/or revision as the patient’s condition requires.

  • Participates in the development and implementation of the patient OASIS assessments, Evaluations, and discharge OASIS documentation.Β 

  • Performs physical therapy treatments as prescribed by the physician, evaluating and treating patients by applying appropriate therapy techniques and modalities in client’s home environment.Β 

  • Recommends modifications or changes in the patients’ Plan of Care based on evaluation of their progress toward the patient goals.

  • Educates/instructs/recommends patient, family members and other health team members of the proper techniques, for exercise program, precautions and self care activities to assure safety for caregivers and patient.

  • Participates in the interdisciplinary team meeting (ITM) by exchanging team information on the progress and discharge plan in order to provide the highest level of continuity of care for the patient.

  • Communicates with client, family members, clients’ advocate(s), caregivers and physicians on patient’s progress and discharge plan on a regular basis.

  • Conducts home visits to implement the patient’s Plan of Care in use of therapeutic and self care activities for the purpose of improving function and safety in order to allow the patient to return home at their highest level of function and independence.

  • Participates in discharge planning by exchanging information with staff members, family members and physician to promote continuity of client care, in accordance with corporate policies, federal, state and local guidelines.

  • Ensures that all work areas, treatment areas and equipment are maintained in a clean, sanitary and safe manner.

Administration and Reporting - 20%

  • Maintains current and accurate patient records through use of personal EMR device, and paper/chart documentation in accordance with company, federal, state, and local guidelines.

People Management and Development - 10%

  • Coaches and influences the work of the Licensed Physical Therapy Assistants (LPTA) regarding the performance expectations of their position.Β 

  • Fosters a positive work environment that attracts, retains, and motivates employees.

  • Interviews, selects, and orients new employees.

  • Defines, establishes, and promotes performance expectations so staff understands and demonstrates excellence through high quality and consistent performance.

  • Evaluates job performance of staff in accordance with the company policy and procedure and provides developmental guidance to staff for improved performance and growth.

  • Monitors accountability with organization requirements prescribed by the Employee Handbook.Β 

  • Participates in the development, planning, scheduling, conducting, and monitoring of in-service training programs, on-the-job training, and orientation programs for departmental staff.

  • May assist to ensure that there is a reasonable and sufficient plan for staffing should a direct report be absent or a position be vacant.

  • Responds timely and thoughtfully to questions, requests, and work product from departmental staff that has been provided for review and approval.

Quality, Compliance and Risk Management - 10%

  • Maintains compliance with all standards, regulations and rules pertinent to services provided.

  • Respects confidential information and follows all company policies and procedures.

Qualifications

Education

  • Current unencumbered license for the state of Ohio to practice as a Physical Therapist (PT) required.

Experience

  • One year experience as a Physical Therapist in a hospital, long-term care facility, or home health agency preferred. New graduates welcome.

  • Experience in the care of geriatric and physical therapy patients required.

  • Knowledge of treatment modalities.

Other Requirements

  • Must be able to read, write, speak, and understand the English language.

  • Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
