Summary





  • JOB SUMMARY/PURPOSE




    • Provides clinical pharmacy services to ambulatory patients. Provides comprehensive medication management in multiple areas of practice, including but not limited to anticoagulation, perioperative management of anticoagulants and antiplatelets, anemia management with erythropoiesis stimulating agents, diabetes, hypertension, hyperlipidemia, heart failure, COPD, asthma and opioid use disorder. Ensures departmental efficiencies and compliance with regulatory bodies. Instructs other pharmacists, pharmacy residents and pharmacy students.  






  • DEPARTMENT DESCRIPTION




    • The Medication Management Clinic provides comprehensive pharmaceutical care and care coordination to ambulatory patients who are on high-risk medications or who require disease state management.






  • EXPERIENCE/EDUCATION/QUALIFICATIONS




    • Current unencumbered Oregon Pharmacist and Pharmacist Preceptor licensure required.



    • Healthcare Provider Level BLS required within 90 days of hire.



    • Previous ambulatory care clinic experience preferred. Alternatives may include completion of a PGY 1 residency with rotations in ambulatory care and anticoagulation rotation, completion of a PGY 2 residency in ambulatory care, or board certification in ambulatory care.



    • All job offers are contingent upon the candidate passing a pre-employment/post offer drug screen.

    • Computer Literacy - Knowledge of electronic equipment and computer hardware, software, and applications. Ability to operate applications, enter data, and process information.




    • Social Perceptiveness - Ability to work with a diverse population from pediatrics to geriatrics. Basic understanding of age-related differences in caring for and/or communicating with patients and caregivers. Possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles and ages. 


    • Team Building - Ability to work, function and communicate on a multi-disciplinary team. Possess the knowledge and skills to develop constructive and cooperative working relationships with others and maintain them over time.
