As a Mental Health Therapist, you will play a vital role in supporting students’ academic, social, and emotional success. Through individual and group counseling, as well as close collaboration with educators, families, and community partners, you will help create a safe, supportive learning environment that strengthens connections between home and school.
‖ Responsibilities Include:
Providing individual and group counseling sessions to address interpersonal, social, emotional, or family challenges impacting school performance, including crisis intervention when necessary.
Conducting comprehensive interviews with students, families, and school personnel to assess social, emotional, and behavioral adjustment to school and community life.
Maintaining consistent communication with parents/guardians by informing them of student progress, challenges, and any home-life changes that may influence academic performance.
Developing and updating Individualized Education Plans (IEPs) and Positive Behavior Intervention Plans (PBIPs) in compliance with company, school, state, and federal guidelines.
Attending and actively participating in IEP team meetings, facilitating when required by the district, and engaging in other team meetings to address student or parent concerns.
Collaborating with educators, school staff, and social service agencies to design and implement appropriate support services for students and families.
Providing case management for students, including coordinating parent meetings, facilitating emergency transportation requests, supporting disciplinary processes, and liaising with outside therapists.
Partnering with teachers, parents/guardians, district personnel, and service providers to identify solutions to academic and behavioral challenges, while building strong relationships and promoting a positive organizational image.
Ensuring consistent use of positive teaching strategies by conducting monthly classroom climate observations and monitoring the implementation of IEP and PBIP goals.
Guiding and consulting with teachers on classroom scheduling, data system management, and student transition planning to ensure alignment with IEP and program goals.
Assessing student progress through ongoing review of data collection systems, and training staff to apply results-driven interventions that address evolving student needs.
Modeling best practices for student engagement, effective communication, and problem-solving with staff—reinforcing a data-driven, behavior-analytic, and collaborative approach to maximize outcomes for students and colleagues.
‖ Qualifications Required:
- Master’s degree or higher in educational counseling, therapy, social work or a closely related field of study.
- Meet all state and district requirements for appropriate licensing and/or credentials.
- Licensed currently or in the process of obtaining one or more of the following credentials:
- LMHC | LPC | LCSW | SSW
- Licensed currently or in the process of obtaining a state special education credential preferred.
- Ability to obtain and maintain certification in company approved crisis management training.
- Prior experience and/or knowledge in special education services and compliance, particularly for students with learning disabilities, emotional behavioral disorders and/or other related disabilities.
- Prior experience and/or highly knowledgeable in providing mental health support to children, preferably in an education and/or behavioral health program setting.
- Well-versed in applied behavior analysis (ABA) including functional behavior assessment (FBA), positive behavior interventions and supports (PBIS) and behavior intervention plans (BIP's).
- Proven success in developing and implementing effective treatment plans for students with diverse needs at various levels.
- Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.
- Ability to think and act quickly and calmly in an emergency and make independent decisions.
- Highly skilled in building relationships with students, parents, teachers and community and/or district partners.
- Advanced oral and written communication, instruction, curriculum, conflict resolution, multitasking, problem solving, and decision making ability.
- Well-versed in the use of relevant technology including experience with computers, Microsoft Office Suite, database entry and basic office equipment.

Responsibilities
Our Occupational Therapists/OTs help patients who have debilitating conditions improve the functions of performing everyday tasks in the home. Occupational Therapists/OTs use treatments to develop the daily living skills of their patients and the basic motor functions of patients, as well as to compensate for any loss of function that patient may be experiencing. Occupational Therapists/OTs administer skilled care to clients requiring intermittent professional services and teach the family and other members of the health care team. These services are performed by Occupational Therapists/OTs in accordance with the physicians' orders and the established plan of care, under the direction and supervision of the Branch Director.
Qualifications
Occupational Therapists/OTs must meet the following requirements:
* Must be a graduate of an approved school of Occupational Therapy and licensed as an Occupational Therapist/OT in the state in which he/she currently practices.
* A minimum of one-year experience is preferred, but can be less with Regional Vice President and/or Regional Director of Therapy Services approval based on prior experience setting.

The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living. We have Specialty Programs that include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement Program, and our Total Shoulder Replacement Program. He/she administers skilled care to clients requiring intermittent professional services and teaches the family and other members of the health care team. Summary:
An LVN/ LPN plays a crucial role in managing patient care and ensuring continuity of services. The Care Coordinator is responsible for making telephonic outreaches to members attributed to our value-based contacts. They support the ACO and CIN network providers and practices in successfully meeting quality improvement initiatives, monitoring standards of care and managing high risk multi morbidity patient populations across CHRISTUS Health ministries. The role focuses on improving quality care gaps, promoting preventive care, and improving patient outcomes.
Responsibilities:
- Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
- Mentor, train and support the team of care coordinators, ensuring high-quality care and adherence to best practices. Assist with work assignments and development of new work processes as needed. Coordinate and assist with associate onboarding. Create education material for training.
- Monitor and ensure compliance with all regulatory requirements, organizational policies, standing delegated orders and protocols.
- Identify quality gaps and risk adjustment gaps. Participate in Quality Improvement Programs as indicated. Attend learning sessions and share information learned with team members. Assist in the development of tools, education, and workflow processes to assist the network in meeting CMS, ACO, documentation, and payor quality initiatives.
- Conducts internal review audits to facilitate feedback for documentation and efficiency of the care coordination team.
- Support Primary Care Providers and assist patients in scheduling preventative screenings and appropriate appointments. Maintain ongoing communication with healthcare providers through various tools and meetings.
- Monitor value-based care quality performance and pulls reports to identify open care gaps. Conducts telephonic outreach on behalf of providers to close care gaps & address medication adherence to facilitate star rating and quality performance.
- Providing counseling and health education to patients and families, using appropriate materials and standardized protocols. Serve as a subject matter expert in care transitions & quality metrics. Assist in educating practice staff on quality, payor, and government program requirements.
- Communicate resources and services available to patients through the continuum of care.
- Escalate health concerns to Primary Care providers and place referrals to appropriate care team members, i.e., Nurse Navigation, CHW, etc. Develop professional working relationships with ACO and CIN network providers, practice managers, and their staff to collaboratively manage follow-up care and improve overall health and wellness. Conduct in-person and virtual meetings with practice managers, staff, providers and managers to communicate program goals, results, and provide education.
- Document relevant, comprehensive information and data using standard assessment tools. Maintain patient chart compliance through proper documentation and updated: preventative screenings, medical history, medication, and immunizations.
- Unburden primary care providers by placing approved orders for labs and other screenings as per the Standing Delegated orders.
- Perform Transition of Care calls on patients transitioning from an inpatient stay to home, or emergency department encounter to identify the need for a follow-up appointment, community resource needs, scheduling follow-up appointments, reviewing discharge instructions, and medications. Utilizing clinical judgment and problem-solving skills to coordinate appropriate care with physicians and Nurse Navigation.
- Prepare and maintain Transitions of Care and Care Management reports and provide periodic updates to network leaders.
- Must have strong leadership, exceptional oral communication skills, strong organizational and analytical skills, ability to adapt to change and motivate a team.
- Must have a strong ability to multi-task and coordinate multiple projects.
- Perform other duties as assigned.
Job Requirements:
Education/Skills
• High School Diploma required.
Experience
• Minimum of 3 years of clinical or home health experience required.
• 5 years supporting value-based care programs, accountable care organizations, or HEDIS
• Knowledge of government programs (CMS), accountable care organizations (ACOs), HEDIS, and experience with payor cost sharing initiatives preferred.
• Knowledge of physician office practice operations and 3 years of experience in a physician practice is preferred.
• Proficiency in keyboarding and EHR systems, primarily Epic.
Licenses, Registrations, or Certifications
• LVN/ LPN in the state of employment and/or compact licensure required.
In accordance with the CHRISTUS Health License, Certification and Registration Verification Policy, all Associates are required to obtain the required certifications for their respective positions within the designated time frame.

Summary:
The Utilization Review Nurse is responsible for determining the clinical appropriateness of care provided to patients and ensuring proper hospital resource utilization of services. This nurse is responsible for performing a variety of pre-admission, concurrent, and retrospective UM related reviews and functions. They must competently and accurately utilize approved screening criteria (InterQual/MCG/Centers for Medicare and Medicaid Services “CMS” Inpatient List). - Protect the confidentiality of data and intellectual property; assures compliance with national health information guidelines.
- Analyze clinical information submitted by medical providers to evaluate the medical necessity, appropriateness, and efficiency of the use of medical services, procedures, and facilities.

POSITION SUMMARY/RESPONSIBILITIES

EDUCATION/EXPERIENCE
Must have completed training in an ACGME-approved and accredited residency program. Responsibilities
Ensure the overall coordination of home health services provided to the patient is delivered according to acceptable standards of practice and company procedures. Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit. Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees.
Qualifications
* Must be a graduate of an approved school of professional nursing.
* Must be licensed in the state in which they currently practice, or in accordance with the board of nursing rules for nurse licensure compact for the state in which they practice.
* Must have at least two years of nursing experience.
* Must have one year experience in home health or hospice.
* Must have demonstrated knowledge and understanding of the federal, state, and local laws and regulatory guidelines that govern a home care operation.
* Must have basic demonstrated technology skills, including operation of a mobile device.
Education and experience, preferred
* Previous experience in management is preferred.

* Must maintain automobile liability insurance as required by law
* Must maintain dependable transportation in good working condition

Job Summary
The Licensed Practical Nurse (LPN) participate in the implementation and evaluation of patient care for stable and predictable patients of all ages in the home care setting. Demonstrates knowledge of nursing principles for the patient in the relevant setting and possesses the ability to perform testing to assess data reflective of the patient's status and to interpret appropriately the information needed to identify each patient's requirements relative to the age-specific needs, and to provide the care needed. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the home care setting. Ensures the health, comfort and safety of patients.
Knowledge of the principles and skills needed for nursing to provide patient care and treatment. Knowledge of medications and their effects on patients. Knowledge of patient education principles to develop a self-care program. Knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting.
Skill in administering a variety of treatments and medications as directed. Skill in documenting and maintaining records. Skill in establishing and maintaining effective working relationships with patients, health care providers, and the public. Ability to recognize problems and recommend solutions. Ability to interpret, adapt, and follow guidelines and procedures. Ability to communicate clearly. Ability to maintain confidentiality of sensitive information. Counsels patients and family members, under the direction of a registered nurse, on prevention and treatment plans.
Qualifications
Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.
One year of professional nursing experience preferred.
Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Valid driver's license required.
