/n/nJob Requirements / Qualifications/n

/n
- Active Registered Nurse (RN) license (state-appropriate licensure required for travel nursing assignment)/n
- BLS Certification (required prior to first shift)/n
- ACLS Certification (required prior to first shift)/n
- Epic EMR experience (electronic medical record proficiency)/n
- Minimum 1 year of bedside clinical experience (2 years highly desired) — LTAC or acute care experience preferred/n

/n/nResponsibilities/n

/n
- Perform comprehensive nursing assessments within 8 hours of patient admission to develop individualized long-term acute care plans/n
- Collaborate with interdisciplinary teams and families to identify patient teaching needs and rehabilitation goals/n
- Provide direct patient care using the nursing process: assessment, planning, implementation, and evaluation/n
- Document assessments, interventions, and patient responses accurately and in a timely manner in Epic EMR/n
- Participate in patient rehabilitation programs and initiate discharge planning from admission/n
- Communicate changes in patient status to providers to maintain continuity of care and patient safety/n
- Supervise and provide direction to ancillary nursing personnel and support staff/n
- Promote patient and employee safety by adhering to hospital policies, procedures, and LTAC clinical protocols/n
- Contribute positively to team building and a supportive work environment on the night shift/n
- Assume charge responsibilities as needed after orientation/n
- Engage in ongoing professional development and continuing education relevant to long-term acute care nursing/n
