Position Overview
The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

• Responsible for the delivery and coordination of quality patient care in compliance with physician orders.
• Continuously observes and assesses patient condition and care needs and reports changes in condition to the supervisor and/or physician as appropriate.
• Documents all activities, assessments, nursing actions, responses and coordination of care in a timely manner whenever care is delivered.
• Participate, implement and update the nursing care plan.

• Recognizes changes in patient needs and responses requiring intervention and implements care to prevent risk or reduce risk.

Requirements
• Graduate of an accredited school of nursing.
• Current, unrestricted state license as a Licensed Nurse in the state of practice

Additional state specific requirements:
• South Carolina – One (1) year of pediatrics experience
• California – One (1) year of experience required working under current nursing license

• Continuing Education as required by state

Chamberlain College of Nursing needs qualified Mental Health Clinical Faculty (part-time) in our Bachelors of Science in Nursing program in Hawaii including Maui, Oahu, Lanai and Kauai.

Teaching is onsite (in clinical setting) with one of our clinical partners. Schedule: most commonly 12-hour rotations (7a-7p) for three to four consecutive days.

Mental-Health Nursing course overview:

Emphasis is on the dynamics of an individual's ability to function in society. The course focuses on content relative to anxiety, self-concept, thought disorders, mood alterations, addictive behaviors, organic brain dysfunction, abuse and violence issues. It also incorporates health promotion and wellness issues such as stress management and personal growth. In this role, the clinical instructor:

Chamberlain Visiting Professor Position - Campus
In support of the pay transparency laws enacted across the country, the expected compensation rate for this position is between $560 per assignment to $7,245 per assignment, depending on the credit hour assigned to the course.

Essential Functions:
1. Compliance with rules and regulations of all federal, state, and local governments is a requirement of employment.
2. Participates in the development of the plan of treatment.
3. Participates in team and interdisciplinary conferences as needed.
4. Plans constructively with resident and family to restore resident to optimum capacity through recreational activities.

7. Implements a planned program of activities to include group/independent programs for all residents at various times of day and evening, seven days a week, which requires weekends and up to three evenings per week. (Holiday schedule will be determined by management.)
8. Assists in the development of a care plan for each resident.
9. Assists Director of Activities in efforts to orient, train, and supervise volunteers.
10. Keeps Activity Director informed of problems, needs, and resident progress.
11. Attends meetings as required such as committee meetings and resident care conferences.

Job Summary

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. Qualifications

Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing is preferred.

If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.

Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as LPN at another facility. Essential Functions: The purpose of an activities program is to create an environment that is as near to normal as possible. The activities program provides physical, intellectual, social, spiritual, and emotional challenges in a planned, coordinated and structured manner. Organize recruitment, interviewing, selection, and orientation of volunteers for the activity department. Assure that volunteers attend and participate in all training programs deemed necessary for setting their orientation to facility. Confirm that all volunteers are knowledgeable of the resident's rights. Organize volunteer activities and enlist volunteers to participate in activity functions. Function as a liaison for the facility in relating to organizations and individuals outside of the hospital. Organize seasonal/holiday events for residents/staff. Place orders for equipment and supplies as necessary and assure that adequate provisions are available and in proper working condition. Assist in preparing and planning the activity department's budget. Participate in resident council meetings and provide support services to such council. Serve on, participate in, and attend Care Plan, Quality Assurance, and interdepartmental supervisors meetings as appointed by the Administrator. Develop and maintain a varied activity calendar monthly. Plan regular bedside, individual and group activities for residents and/or family members to develop preliminary and comprehensive assessments for the activity needs of each resident. Interview residents and/or family members to develop preliminary and comprehensive assessments of the activity needs of each resident. Participates in the interdisciplinary team meetings to develop a written plan of care for each resident that identifies the problems/needs of the resident and the goals to be accomplished. Coordinate activities with other departments as necessary. Schedule sufficient religious activities from varied denominations to ensure that residents are ensured religious freedom and can participate in religious activities. Organizes adequate resources to fulfill the activities function (games, movies, classes, etc.) drawing on a many resources outside the facility as possible. Coordinates and Supervises the Volunteer and Chaplin programs with the awareness of Guthrie relevant policies and processes. Document each resident's progress and attendance according to initial assessment and resident's total plan of care. Review and revise care plans as necessary, but at least quarterly. Participate in surveys made by authorized government agencies.
Other Duties: Work closely with local news media and the public, and participate in community planning as related to the interests of the facility. Arrange transportation for field trips when necessary. Educate other facility staff of resident's activity plans in order for them to assist with activities, particularly in the absence of the Activities Director. Attends and participates in continuing educational programs, job‐related in‐service and conferences. Ensures residents' personal property rights. Others as deemed necessary and appropriate, or as may be directed by the Administrator.

In this role, you will:

  • Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area

  • Primary coverage areas include but are not limited to: Springfield MA

  • Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month

  • Execute a strong visit completion rate, pacing 35+ completed visits per month

You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.
