Minimum Qualifications
- Masterβs degree from an accredited college or university in Hospital, Health, Public or Business Administration, Healthcare Management, Public Health or a related discipline; and four (4) years of full-time, paid progressively responsible experience in hospital administration with an emphasis on the development and evaluation of health care delivery services; or
- Bachelorβs degree from an accredited college or university in disciplines, as listed in #1 above; and five (5) years of full-time, paid experience, as described in #1 above.

In this role, you will:
Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area

Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
Execute a strong visit completion rate, pacing 35+ completed visits per month

During our In-Home Health Evaluations (and other associated follow-up visits), you'll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.
The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.
