As a Center Therapy Director, you will be responsible for providing treatment programs to restore and improve the physical functions of our patients. You will design programs based on the patient's specific goals to get them back to work and life with dignity and confidence. Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Assists in monitoring patient progress through observations and follows up with supervising therapist. Works under PT supervision as required under NY State Education law.

• Participates in VNS Health meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
• Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
• Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment.
• Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care.

Essential Job Functions:
- Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
- Responsible for administration, evaluation, and interpretation of tests and measurements to determine functional goals and outcomes.
- Provide a written evaluation and plan of care defining physical therapy needs, goals, progress and expected outcomes.

Therapy Care Coordinator Responsibilities:

The Physical Therapist Assistant is part of a multi-disciplinary team to deliver patient-centered physical rehabilitation services to patients, supports Physical Therapist in treatment procedures. 2. Ensures operation of equipment by using equipment consistent with equipment specifications; completing preventive maintenance requirements; following manufacturer¿s instructions; troubleshooting malfunctions; and calling for repairs.
3. Participates and represents PT staff on committees, as requested by supervisors.
4. Demonstrates appropriate knowledge in assessing the need for durable medical equipment for patients. Follows through with appropriate professionals to ensure proper fitting and educates patient on proper usage.
