Job Description
We offer diverse specialized services, including Back Pain, Concussion, Vestibular/Vertigo, Dry Needling, Foot and Ankle, Geriatrics, Head, Neck, and Facial Pain, Health and fitness, Industrial Rehabilitation, Local School Sports, and Manual Therapy. Rehab Associates of Central Virginia’s strong patient population further defines its reputation for delivering effective, personalized healthcare solutions.
- Assesses patients and develops and initiates treatment plans based on the evaluation results.
- Review the Physician's referral (Prescription) and the patient's condition and medical records to determine the physical therapy treatment required.
- Provides direct patient care by established protocols and patient care guidelines.
- Documents findings, progress, and instructions to patients and caregivers.
- Responsible for patient care performed by assistants and rehabilitation technicians.
- Completes a written plan of care, develops goals based on evaluation findings, and develops a comprehensive program to attain goals.
- Implements physical therapy treatment program and provides essential physical therapy treatments.
- Communicates effectively with patients/caregivers and professional colleagues, including physicians and insurance companies.
- Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports.
Qualifications
- Graduate from CAPTE-accredited Physical Therapy program.
- Current state of Virginia license or ability to obtain Virginia license and CPR certification.
- Able to create and implement outpatient balance and vestibular rehabilitation-setting treatment plans.

Responsibilities
Home Health with Less Travel!
Our Physical Therapists (PT) supervise, evaluate, and provide therapy services to clients in various senior living communities, including assisted living and independent living facilities.
Our facility-based PTs enjoy:
* providing therapy services to our patients within the beautiful confines of the assisted living facilities where our patients reside

Our talented and highly skilled Physical Therapists work collaboratively with each assisted living facility's wellness team on a daily basis to maintain strong lines of communication and overall continuity of care for the residents and their loved ones. Enhabit PTs examine and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independence in self-care, and other skills necessary for functioning in daily living and to successfully age in place. Enhabit Home Health & Hospice offers our patients Specialty Programs that include Balance and Fall Prevention, Spine Safety, Low Vision, and Diabetic Management programs to help them achieve their goals. Our Physical Therapists administer skilled care to clients requiring intermittent professional services and teach the family and other members of the health care team. These services are performed in accordance with the physician's orders and the established plan of care, under the direction and supervision of the Branch Director.
Qualifications
Physical Therapists / PTs must meet the following requirements:

Summary:
The Physical Therapist has the responsibility and accountability for assessing, planning, evaluating and implementing care for the patients assigned physical therapy. The Physical Therapist is responsible for adhering to all standards of Physical Therapy Practice Act as they apply to providing technical therapy care, supplies, equipment and interventions to a designated patient population.
This position requires providing services to Therapy Patients, ages between children/adolescents to geriatrics in a manner that demonstrates an understanding of the functional/developmental age of the individuals served. It is expected that the associate demonstrate behavior consistent with the Core Values. The associate shall support the CHRISTUS St. Patrick’s strategic plan.
May require push/pull/lifting of 50-100 pounds. May require partial lift or supporting of weight from 100-150 pounds during transfers or assisting patients in stance. Work requires sitting, bending, stretching, and walking. May require prolonged standing and walking, lifting and moving patients, equipment and supplies. - Patient Care – Completes evaluation and administers patient care as specified by the treatment plan and goals. Productivity adheres to the hospital standard. Flexible and adaptive to changes in the work environment/function. Uses resources in a cost effective manner.
- Records Progress – Records patient evaluations, treatment documentation, discharge documentation, education documentation, billing in a timely manner, maintaining accuracy and updating as appropriate.
- Safety/Infection Control – follows all safety and infection control policies. Uses appropriate therapeutic delivery. Recognizes adverse reactions and takes steps to remedy them.
- Insures Quality – Insures a quality approach in the treatment of patients. Communicates effectively and regularly with medical staff, department directors and Administration, as well as other departments and outside stakeholders as necessary and appropriate. Seeks consults from other disciplines when indicated. Assists in PT/OT/ST scheduling coordination.
- Accepts feedback and modifies actions as necessary. Actively seeks methods to improve performance. Seeks opportunity for personal/professional growth.
