Definition
Under general and/or immediate supervision from higher level engineering staff, performs a variety of journey level technical drafting, office and field engineering work; and provides assistance and information to the public. May exercise functional and technical supervision over less experienced technical staff.
Position Snapshot/A Day in the Life: The Engineering Technician II performs routine drafting and technical engineering work in support of municipal infrastructure projects. Responsibilities include assisting with engineering design and planning, providing customer service, and performing technical office and field duties related to updating, maintaining, and analyzing the Public Works and Engineering Department's Geographic Information System (GIS) and utility infrastructure data. This position coordinates and conducts research, field data collection, and condition assessments of infrastructure assets, using GIS, database, and information management tools to support capital planning and ensure compliance with regulatory requirements.
Essential Functions
Engineering Technician II -This is the full journey level in the Engineering Technician series. Employees within this class are distinguished from the Engineering Technician I By the performance of the full range of duties as assigned including the moderately complex duties of drafting, office and field engineering work. Employees at this level receive only occasional instruction or assistance as new or unusual situations arise, and are fully aware of the operating procedures and policies of the work unit. This class is flexibly staffed and is normally filled by advancement from the I level, or when filled from the outside, requires previous work experience.
ESSENTIAL FUNCTIONS:Incumbent must have the ability to:
- Review and process tract maps, parcel maps, of line adjustments, grants of easement, and street right-of-ways for recordation.
- Verify fulfillment of conditions of approval for new subdivisions; perform on-site inspections to verify survey monuments related to parcel maps and lot line adjustments.
- Prepare and draft technical engineering plans for construction of streets, alleys, storm drains, water lines, and other public improvements.
- Provide public assistance at counter and over the telephone.
- Prepare quantity and cost estimates for engineering projects.
- Maintain a variety of maps and records including tract maps, parcel maps, lot line adjustments, easements, street and address changes.
- Review engineering plans to determine appropriate layout of construction projects.
- Perform on-site inspections to obtain information; determine angles and set grade stakes, hubs, turning points and benchmarks using appropriate surveying techniques; locate and measure boundary lines, right-of-ways, and land grades.
- Make and check mathematical calculations related to surveying, drafting and basic engineering.
- Maintain accurate survey records in the form of field notes and sketches.
- Participate in a variety of engineering design and planning work related to municipal engineering projects.
- Answer questions and provide information to the public concerning engineering activities.
- Assist in development and maintenance of the record keeping and filing systems.
- Prepare, assemble and distribute copies of maps, charts and blueprints.

WORKING CONDITIONS: Position requires prolonged sitting, standing, walking, reaching, twisting, turning, kneeling, bending, and stooping in the performance of daily activities. The position also requires grasping, repetitive hand movement and fine coordination in preparing reports using a computer keyboard. Additionally, the position requires near and far vision when preparing and reading written reports and other work related documents. Acute hearing is also required when providing phone and counter assistance.

Engineering Technician II - Two years of increasingly responsible experience performing drafting and technical engineering work.
Education: Equivalent to completion of the twelfth grade supplemented by specialized training in drafting, surveying or a related field.

We are seeking a Program Manager Pathways to join our team. Your role is responsible for fostering an environment that promotes the well-being, personal growth, and healing of youth. You will provide leadership and strategic direction to a team supporting youth in transitional living, with the goal of equipping them with the skills, confidence, and resources necessary for a successful transition into adulthood and independent living. As a member of the Pathways - MO team, you will work with other team members and report to our Senior Director of Transitional Living.
WHAT YOU WILL DO:
- Implement and maintain a Trauma Informed culture within the Pathways program that aligns with The Sanctuary Model
- Conduct regular supervision of and consultation with Case Managers and Transitional Youth Behavioral Health Specialists regarding daily job duties and work performance
- Conduct timely and accurate annual written performance evaluation of each team member supervised
- Monitor and track staff attendance and approve time off requests
- Organize and implement the independent living skills classes and activities in a variety of formats to meet the needs of youth
WHAT YOU WILL BRING:
Our ideal candidate will have 3-5 years of experience working with vulnerable populations,
- Bachelor's degree in social work or related field. Master's degree is preferred.
- 2 years of supervisory experience working with staff in a youth care, transitional living program, congregate housing or educational setting, preferred.
- At least 21 years of age and pass background check, physical, and drug screening.
- A valid driver's license in the state you reside in, proof of current vehicle insurance, and reliable transportation.

CORE JOB DUTIES/RESPONSIBILITIES:
- Manages and plans for transitions of care, discharge and post discharge follow-up for patients admitted to key, high-volume/high-priority hospitals.
- Establishes a trusting relationship with patients and their caregivers.
- Collaborates with clinical staff in the development and execution of the plan of care and achievement of goals. Reports variations to PCP/Transitional Care Physicians (TCP) and implements actions as appropriate.
- Builds relationships with preferred acute care providers (hospitalists, specialists, etc.).
- Directs referrals to preferred providers.
- Coordinates the integration of social services/case management functions in the pre-acute, ER, acute and post-acute setting. Coordinates the patient care, discharge and home planning processes with hospital case management departments, and other healthcare facilities.
- In conjunction with the PCP, Hospitalist, Medical Director, insurance case manager and the hospital case manager, coordinates the patient transition to the appropriate/least constrictive level of care using a preferred provider.
- Keeps the PCP aware of patient(s) condition via e-mail, DASH, HITS or other appropriate means of communication.
- Introduces self to patient/family and explains Nurse Case Managers role and processes to contact the Nurse Case Manager for questions, guidance and education.
- Provides high intensity engagement with patient and family.
- Facilitates patient/family conferences to review treatment goals and optimize resource utilization; provides family education and identifies post-hospital needs.
- Serves as a patient advocate. Enhances a collaborative relationship to maximize the patient/familys ability to make informed decisions.
- Addresses advanced care planning including treatment goals and advance directives.
- Refers cases to social worker (Hospital and ChenMed/JenCare/Dedicated) for complex psychosocial and economic needs.
- Refers cases where patient and/or family would benefit from counseling required to complete complex discharge plan to social worker.
- Reports observed or suspected child or adult abuse pursuant to mandated requirements.
- Obtains onsite and EMR access at priority facilities.
- Maintains clinical and progress notes for each patient receiving care and provides progress report to PCP and others as appropriate.
- Submits required documentation in a timely manner and in appropriate computer system.
- Participates in surveys, studies and special projects as assigned.
- Conducts concurrent medical record review using specific indicators and criteria as approved by medical staff. Acts as patient advocate: investigates and reports adverse occurrences, and performs staff education related to resource utilization, discharge planning and psychosocial aspects of healthcare delivery.
- Promotes effective and efficient utilization of clinical resources and mobilizes resources to assist in achieving desired clinical outcomes within specific timeframe.
- Conducts review for appropriate utilization of services from admission through discharge. Evaluates patient satisfaction and quality of care provided.
- Communicates with physicians at regular intervals throughout hospitalization and develops an effective working relationship. Assists physicians to maintain appropriate cost, case and desired patient outcomes.
- Coordinates the provision of social services to patients, families and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from healthcare services.
- Completes expanded assessment of patients and family needs at time of admission. Completes psychosocial assessment.
- Directs and participates in the development and implementation of patient care policies and protocols to provide advice and guidance in handling unusual cases or patient needs.
- Attends meetings as assigned
- Performs other duties as assigned and modified at managers discretion.
There are 4 Nurse Case Manager 1 Roles with additional Essential Job Functions:
Acute Case Manager (primarily hospital based)
Responsibilities include all the above Core duties/responsibilities plus the following:
- Identify appropriateness of inpatient vs. observation status.
- Identify and manage safety risk (complete a social assessment), identify functional status (ADLs and PT needs), discuss medications and self-management, identify and correct knowledge deficits.
- Implement the ACM Coaching program with the appropriate patient population.
- In markets as appropriate, when patient in SNF, in conjunction with the post-acute physician, coordinate the transition to a lower level of care as soon as appropriate using a preferred provider if further services are needed.
- Facilitate discharge to appropriate level of care and preferred providers
- Communicate discharge to all stakeholders including PCP, Center Manager and Community Case Manager.
- Document the appropriate date that the patient is medically discharged and update as appropriate.
- Contact the center manager to arrange for a follow-up PCP appointment prior to discharge and whenever possible, communicate this information to the patient/caregiver.
- As appropriate, discuss patients eligibility for CCM or DM programs and identify patient interest in participation.
- Coordinate acute UR physician meetings.
Community Case Manager (primarily clinic and community based)
Responsibilities include all the above Core duties/responsibilities plus the following:
- Provides telephonic or outpatient visits to patients at high-risk for readmissions (as identified by CM Plan) to the ER or hospital, to patients with active care planning requirements, to disease management patients per the Disease Management Plan and to others as referred via transitional care team, acute case managers and Transitional Care team.
- Visits may include evening and weekend hours with the goal of preventing ER visits or hospital admissions.
- Performs clinical functions including disease-oriented assessment and monitoring, medication monitoring, health education and self-care instructions in the outpatient setting.
Coordinate the Plan of Care:
- Conducts/coordinates initial case management assessment of patients to determine outpatient needs.
- Ensures individual plan of care reflects patient needs and services available.
- Makes recommendations to the team.
- Completes individual plan of care with patients and team members.

- Maintains ongoing communication with families, community providers and others as needed to promote the health and well-being of patients.

- Monitors the quality, frequency and appropriateness of HHA visits and other outpatient services.

- CM telephonic or onsite visits to SNFs, communication with physical therapists (PT), social workers, patient and families as appropriate.
- Validates appropriate level of care/LOS.

- Reminds patient of need for 4-day PCP post hospital/SNF discharge visit and future visits.
- Collaborates with payor onsite SNF CMs.

CORE JOB DUTIES/RESPONSIBILITIES:
- Manages and plans for transitions of care, discharge and post discharge follow-up for patients admitted to key, high-volume/high-priority hospitals.
- Establishes a trusting relationship with patients and their caregivers.
- Collaborates with clinical staff in the development and execution of the plan of care and achievement of goals. Reports variations to PCP/Transitional Care Physicians (TCP) and implements actions as appropriate.
- Builds relationships with preferred acute care providers (hospitalists, specialists, etc.).
- Directs referrals to preferred providers.
- Coordinates the integration of social services/case management functions in the pre-acute, ER, acute and post-acute setting. Coordinates the patient care, discharge and home planning processes with hospital case management departments, and other healthcare facilities.
- In conjunction with the PCP, Hospitalist, Medical Director, insurance case manager and the hospital case manager, coordinates the patient transition to the appropriate/least constrictive level of care using a preferred provider.
- Keeps the PCP aware of patient(s) condition via e-mail, DASH, HITS or other appropriate means of communication.
- Introduces self to patient/family and explains Nurse Case Managers role and processes to contact the Nurse Case Manager for questions, guidance and education.
- Provides high intensity engagement with patient and family.
- Facilitates patient/family conferences to review treatment goals and optimize resource utilization; provides family education and identifies post-hospital needs.
- Serves as a patient advocate. Enhances a collaborative relationship to maximize the patient/familys ability to make informed decisions.
- Addresses advanced care planning including treatment goals and advance directives.
- Refers cases to social worker (Hospital and ChenMed/JenCare/Dedicated) for complex psychosocial and economic needs.
- Refers cases where patient and/or family would benefit from counseling required to complete complex discharge plan to social worker.
- Reports observed or suspected child or adult abuse pursuant to mandated requirements.
- Obtains onsite and EMR access at priority facilities.
- Maintains clinical and progress notes for each patient receiving care and provides progress report to PCP and others as appropriate.
- Submits required documentation in a timely manner and in appropriate computer system.
- Participates in surveys, studies and special projects as assigned.
- Conducts concurrent medical record review using specific indicators and criteria as approved by medical staff. Acts as patient advocate: investigates and reports adverse occurrences, and performs staff education related to resource utilization, discharge planning and psychosocial aspects of healthcare delivery.
- Promotes effective and efficient utilization of clinical resources and mobilizes resources to assist in achieving desired clinical outcomes within specific timeframe.
- Conducts review for appropriate utilization of services from admission through discharge. Evaluates patient satisfaction and quality of care provided.
- Communicates with physicians at regular intervals throughout hospitalization and develops an effective working relationship. Assists physicians to maintain appropriate cost, case and desired patient outcomes.
- Coordinates the provision of social services to patients, families and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from healthcare services.
- Completes expanded assessment of patients and family needs at time of admission. Completes psychosocial assessment.
- Directs and participates in the development and implementation of patient care policies and protocols to provide advice and guidance in handling unusual cases or patient needs.
- Attends meetings as assigned
- Performs other duties as assigned and modified at managers discretion.
There are 4 Nurse Case Manager 1 Roles with additional Essential Job Functions:
Acute Case Manager (primarily hospital based)
Responsibilities include all the above Core duties/responsibilities plus the following:
- Identify appropriateness of inpatient vs. observation status.
- Identify and manage safety risk (complete a social assessment), identify functional status (ADLs and PT needs), discuss medications and self-management, identify and correct knowledge deficits.
- Implement the ACM Coaching program with the appropriate patient population.
- In markets as appropriate, when patient in SNF, in conjunction with the post-acute physician, coordinate the transition to a lower level of care as soon as appropriate using a preferred provider if further services are needed.
- Facilitate discharge to appropriate level of care and preferred providers
- Communicate discharge to all stakeholders including PCP, Center Manager and Community Case Manager.
- Document the appropriate date that the patient is medically discharged and update as appropriate.
- Contact the center manager to arrange for a follow-up PCP appointment prior to discharge and whenever possible, communicate this information to the patient/caregiver.
- As appropriate, discuss patients eligibility for CCM or DM programs and identify patient interest in participation.
- Coordinate acute UR physician meetings.
Community Case Manager (primarily clinic and community based)
Responsibilities include all the above Core duties/responsibilities plus the following:
- Provides telephonic or outpatient visits to patients at high-risk for readmissions (as identified by CM Plan) to the ER or hospital, to patients with active care planning requirements, to disease management patients per the Disease Management Plan and to others as referred via transitional care team, acute case managers and Transitional Care team.
- Visits may include evening and weekend hours with the goal of preventing ER visits or hospital admissions.
- Performs clinical functions including disease-oriented assessment and monitoring, medication monitoring, health education and self-care instructions in the outpatient setting.
Coordinate the Plan of Care:
- Conducts/coordinates initial case management assessment of patients to determine outpatient needs.
- Ensures individual plan of care reflects patient needs and services available.
- Makes recommendations to the team.
- Completes individual plan of care with patients and team members.
- Communicates instructions and methodologies as appropriate to ensure that the plan is implemented correctly.
- Assesses the environment of care, e.g., safety and security.
- Assesses the caregiver capacity and willingness to provide care.
- Assesses patient and caregiver educational needs.
- Coordinates, reports, documents and follows-up on Super Huddles and HPP/IDT meetings.
- Helps patients navigate health care systems, connecting them with community resources; orchestrates multiple facets of health care delivery and assists with administrative and logistical tasks.
- Coordinates the delivery of services to effectively address patient needs.
- Facilitates and coaches patients in using natural supports and mainstream community resources to address supportive needs.
- Maintains ongoing communication with families, community providers and others as needed to promote the health and well-being of patients.
- Establishes a supportive and motivational relationship with patients that support patient self-management
- Monitors the quality, frequency and appropriateness of HHA visits and other outpatient services.
- Assists patient and family with access to community/financial resources and refer cases to social worker as appropriate.
Community/Skilled Nursing Facility Case Manager (Community Case Manager Role with additional SNF duties as assigned)
Responsibilities include all the above Core duties/responsibilities plus the following:
- Community Case Manager role as above.
- CM telephonic or onsite visits to SNFs, communication with physical therapists (PT), social workers, patient and families as appropriate.
- Validates appropriate level of care/LOS.
- Validates Discharge plan for safe transition home, utilization of preferred providers or timely transition to long term care.
- Reminds patient of need for 4-day PCP post hospital/SNF discharge visit and future visits.
- Collaborates with payor onsite SNF CMs.
Transitional Case Manager (Blended Acute and Community Case Manager Roles)
Responsibilities include all the above Core duties/responsibilities plus the following:
- Acute and Community Case Manager roles as above.

PAY RANGE:
$36.9 - $52.70 Hourly
The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for a bonuses or commissions.

Job DescriptionGeneral job duties for all store team members include:
- Operate all equipment.
- Stock ingredients from delivery area to storage, work area, walk-in cooler.
- Prepare product.

Varying and sometimes adverse weather conditions when delivering product, driving and couponing.
Sensing: Talking and hearing on telephone. Near and mid-range vision for most in-store tasks. Depth perception. Ability to differentiate between hot and cold surfaces. Far vision and night vision for driving.
Temperaments: The ability to direct activities, perform repetitive tasks, work alone and with others, work under stress, meet strict quality control standards, deal with people, analyze and compile data, make judgements and decisions.
QualificationsJob requirements include: You must be at least 18 years of age and have a valid driver's license with a safe driving record meeting company standards as well as access to an insured vehicle which can be used for delivery. You should possess navigational skills to read a map, locate addresses within designated delivery area and must be able to navigate adverse terrain including multi-story buildings.
Additional InformationPhysical Requirements include, but not limited to the following:
Standing: Most tasks are performed from a standing position. Walking surfaces include ceramic tile "bricks" with linoleum in some food process areas. Height of work surfaces is between 36" and 48".
Walking: For short distances for short durations. Delivery personnel must travel between the store and delivery vehicle and from the delivery vehicle to the customer's location.
Sitting: Paperwork is normally completed in an office at a desk or table.
Lifting: Bulk product deliveries are made twice a week or more and are unloaded by the team member using a hand truck.
Deliveries may include cases of ingredients and supplies weighing up to 50 pounds with dimensions of up to 3' x 1.5'.
Cases are usually lifted from floor and stacked onto shelves up to 72" high.
Carrying: Large cans, weighing 3 pounds, 7 ounces, are carried from the workstation to storage shelves.
Occasionally, pizza sauce weighing 30 pounds is carried from the storage room to the front of the store.
Trays of pizza dough are carried three at a time over short distances, and weigh approximately 12 pounds per tray.
During delivery, carry pizzas and beverages while performing "walking" and "climbing" duties.
Pushing: To move trays which are placed on dollies.
A stack of trays on a dolly is approximately 24" - 30" and requires a force of up to 7.5 pounds to push.
Trays may also be pulled.
Climbing: Team members must infrequently navigate stairs or climb a ladder to change prices on signs, wash walls, perform maintenance.
During delivery of product, navigation of five or more flights of stairs may be required.
Stooping/Bending: Forward bending at the waist is necessary at the pizza assembly station.
Toe room is present, but workers are unable to flex their knees while standing at this station.
Duration of this position is approximately 30 - 45 seconds at one time, repeated continuously during the day.
Forward bending is also present at the front counter and when stocking ingredients.
Crouching/Squatting: Performed occasionally to stock shelves and to clean low areas.
Reaching: Reaching is performed continuously; up, down and forward.
Workers reach above 72" occasionally to turn on/off oven controls, change prices on sign, and lift and lower objects to and from shelves.
Workers reaching down to perform such tasks as scooping cornmeal from a plastic barrel, or washing dishes.
Workers reach forward when obtaining topping ingredients, cleaning work surfaces, or answering phones.
Driving: Deliver pizzas within a designated delivery area. A team member may make several deliveries per shift.
Deliver flyers and door hangers.
Requires: Valid driver's license with safe driving record meeting company standards. Access to insured vehicle which can be used for delivery.

About You
Job Summary
We are looking for an experienced SAP Integration Developer to design, develop, and maintain robust integration solutions between SAP systems and various internal and external applications. The ideal candidate will have deep knowledge of SAP integration tools such as SAP PI/PO, SAP CPI (Cloud Platform Integration), BTP, IDocs, BAPIs, RFCs, and APIs. This role is critical for ensuring data consistency, data exchange, system efficiency, and seamless business process execution across enterprise applications
Key Responsibilities
- Design and implement secure, scalable integration solutions to enable data exchange between SAP and non-SAP systems, including legacy applications and external platforms
- Work closely with business analysts, functional consultants, and end-users to understand business requirements and translate them into technical specifications
- Build and configure integration flows, mappings, APIs, and other components using SAP's CIA and PIPO middleware tools. This often involves using protocols like REST, SOAP, SFTP, and OData
- Work with third-party vendors and external systems to implement external integrations (e.g., logistics, finance, BI systems)
- Create test plans, execute test cases (unit and integration), and validate test data to ensure the quality and reliability of all integration processes
- Diagnose and resolve technical issues and performance bottlenecks in existing interfaces, providing ongoing maintenance and support

- Configure and customize of PI-Basis functions like setting up of RFC Destinations, Port and Partner Profiles
- Should be proficient in developing Enterprise System Repository (ESR) and Interface Registry (IR) objects, Graphical and Java mapping and proficient on XML Technologies.
- Experience in developing / configuring SAP PI BPMs, Web Services and SOA scenario Experience with Java Development, Java Maps and XML

Compensation and Benefits
The salary range for this position is $120,000 to $175,000. Placement in the range depends on several factors, including experience, skills, education, geography, and budget considerations.
