Responsibilities
1. Support all aspects related to Fund Accounting
2. Perform month-end closing activities such as reconciliations and journal entries
3. Coordinate with finance team to complete assigned accounting tasks within deadlines

Job Details

Working onsite at a Leading Hospital in the Brooklyn Borough, the Hospice Care Planner is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services. The Hospice Care Planner will facilitate admission of referrals upon discharge from the hospital to a Hospice program that meets the needs of the patient and family members.

As a key member of the business development and marketing team, the Hospice Care Planner develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.

Full time role offers a 4 day work week with (10) hour shifts. MUST work 2 full weekends per month (Saturday and Sunday)

  • Serve as a key intermediary between hospital staff, patients, and their families to ensure clear and effective communication and coordination of hospice care.
  • Conduct thorough chart assessments of patient cases and bedside evaluations to determine the clinical picture and hospice eligibility
  • Collaborate with facility clinical team and hospice physicians to make informed decisions regarding patient care
  • Successfully facilitate numerous hospice admissions ensuring seamless transitions for patients and their families
  • Provide critical support and education to enhance patient and family understanding of hospice care, leading to improved satisfaction and care outcomes
  • Oversee the coordination of admission assessments and the procurement of durable medical equipment, ensuring that all patient needs are met promptly and effectively

Manage caseload of (4) terminally ill Patients per day, generate and follow plan of care to provide patients with Maximum level of comfort and quality of life in home, Skill nursing facilities and In-patient units settings Coordinate care efforts among LPN, HHA and care givers.

  • Makes staffing decisions and adjustments on a shift-by-shift basis in collaboration with the Assistant Nurse Manager, Nursing Supervisor, and Nursing Director as needed
  • Responsible for the planning, development and implementation of the annual budget
  • Develops and implements action plans in response to variances
  • Oversees the pay practices of staff and the systems that support payroll and benefits

Management, Leadership, and Development of Staff:

  • Demonstrates principles of evidence-based leadership

We are seeking an experienced and dynamic Permanent Superintendent Healthcare in the Construction industry to join our progressive team. This role is ideal for a professional with a solid background in construction, particularly in ground up, commercial finishes, finishes, and framing. The successful candidate will have the opportunity to work on a variety of healthcare construction projects, ensuring that all aspects of work are carried out to the highest standard. The Permanent Superintendent Healthcare is responsible for overseeing all stages of project life cycles, from initial planning to completion, ensuring that all work is completed on time, within budget, and in accordance with the highest safety and quality standards.

Responsibilities:

The Permanent Superintendent Healthcare will be responsible for the following:

1. Overseeing the daily operations of healthcare construction projects, ensuring that all work is carried out efficiently and effectively.
2. Collaborating with project managers, architects, engineers, and other construction professionals to ensure that all projects are completed to the highest standard.
3. Ensuring that all work is carried out in accordance with relevant health and safety regulations, as well as company policies and procedures.
4. Managing project schedules, ensuring that all work is completed on time and within budget.
5. Supervising and directing construction teams, providing guidance and support as necessary.
6. Coordinating with contractors, suppliers, and other stakeholders to ensure the smooth running of all projects.
7. Conducting regular site inspections to ensure that all work is being carried out to the highest standard.
8. Resolving any issues or problems that may arise during the construction process, ensuring that all work is completed to the highest standard.

Qualifications:

The ideal candidate for the Permanent Superintendent Healthcare role will have the following qualifications:

1. A minimum of 5 years of experience in the construction industry, with a focus on healthcare construction projects.
2. Proven experience in ground up, commercial finishes, finishes, and framing.
3. Excellent leadership and team management skills, with the ability to motivate and inspire teams to deliver exceptional results.
4. Strong communication and interpersonal skills, with the ability to build strong relationships with a wide range of stakeholders.
5. A strong understanding of health and safety regulations, as well as company policies and procedures.
6. Excellent problem-solving skills, with the ability to resolve issues quickly and effectively.
7. A degree in construction management, engineering, or a related field is preferred.
8. A valid driver’s license and the ability to travel as necessary.
9. Proficiency in project management software and other relevant computer applications.
