What you will do:

  • Provides psychosocial assessment, intervention, evaluation, consultation, and education to patient/families as part of comprehensive care management services.,

  • Identifies high risk patients from a psychosocial/financial perspective and provides information, support, counseling, care management and referrals to appropriate resources.

  • Serves as a liaison for complex transitions/discharges between the hospital and community agencies for the exchange of clinical and referral information.

  • Contributes to training of social work professionals as part of the education program at LUHS.

  • Facilitates and coordinates details of placement and actual discharge of complex discharges to appropriate agencies. Discusses with interdisciplinary team the discharge plan and any barriers to a safe discharge. 

 

What you will need:

  • Master's degree required in Social Work.

  • Licensed Social Worker (LSW) required; LCSW preferred.

  • Excellent communication skills.

  • Ability to manage crisis situations calmly and effectively.

  • Ability to work under stressful conditions and in difficult situations.

Salary Range: $30.00 - $46.51per hour

POSITION PURPOSE

Provides and directs safe, effective, and culturally competent care for patients with acute medical and surgical conditions. Through evidence-based practice, utilizes critical thinking to assess, plan, implement, and evaluate outcome-based care. The Acute Care RN manages a diverse patient population with varying levels of acuity, including post-operative, medical, telemetry, and neurologic patients. 1. Conducts an initial assessment to gather data.

  • Makes an initial assessment of new admissions within 30 minutes of the patient’s arrival to the unit.
  • Completes all areas of the admission assessment database within 24 hours of admission.

2. Identifies patient problems and develops them into an outcome/goal-based plan of care.

  • Initiates the patient problem list and incorporates relevant medical history into the plan of care.
  • Analyzes and interprets the assessment data to identify desired patient outcomes/goals.
  • Determines nursing interventions to achieve those outcomes/goals.
  • Prioritizes patient care needs/ activities.
  • Identifies and makes appropriate referrals (i.e., Patient/Family Services, Nutritional Services, Spiritual Care, Diabetic Educator, etc.), and education needs.
  • Collaborates with other health care disciplines to plan interventions/ patient outcomes/goals.

3. Implements the outcome/goal- based plan of care.

  • Assigns and coordinates patient care incorporating patient acuity and the skills, knowledge and abilities of the team members.

  • Maintains ongoing communication with interdisciplinary team members throughout the shift, at shift change, and transfer of care.

6. Demonstrates clinical competence.

  • Utilizes Patient Care Policies and Procedure as references to guide nursing practice.

POSITION SUMMARY

Cerner PowerChart and Scheduling Analyst who reports directly to the Director of Health Information Technology, Interoperability Services and is responsible for the maintenance of the PowerChart and Scheduling functionality. Responsibilities also include providing immediate application and systems support and overseeing the completion of projects, system enhancements, applying vendor upgrades, application support and support of users.

JOB DUTIES (List in order of importance)

  • Utilizing Cerner PowerChart and Scheduling the analyst will design, build and document application changes. Consult with end users on Scheduling module design and build changes.
