The Clinical Care Manager (CCM) Specialist - Children’s Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care. As a member of CBH’s Complex Care team, training and certification in High-Fidelity Wraparound, an evidence-based care management intervention, will be provided.
The CCM Specialist will also be responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, and treatment quality monitoring. As a member of the Children’s Complex Care team, you will have an opportunity to apply your understanding of behavioral health and your cultural competence to work collaboratively with CBH members, their caregivers, their supports, other professionals, and community partners to screen/assess, and develop individualized, family-anchored, comprehensive, and strength-based plans of care. The CCM Specialist must utilize clinical knowledge of best practices and evidence-based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, CCM Specialists consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized.
As a High-Fidelity Wraparound (HFW) Facilitator, the CCM Specialist assists members and their families in learning to accomplish goals they have identified by exploring the strengths and culture around each need they have. The CCM Specialist facilitates monthly interagency planning meetings to assist the member and their family in planning around and working towards meeting their needs. The CCM Specialist will transfer their Wraparound skills to the member and their family, by teaching them to think outside of the box and use natural and community supports.
Essential Functions:
- Work directly with members, natural supports and providers in the community, including in members’ homes.
- Facilitate care coordination. Using an inter-generational approach, with system partners to address social determinants, barriers to wellness, and other needs of members, including connection to appropriate community-based services.
- Create and maintain a team of system partners and natural supports for each member and their family.
- Complete an effective Crisis Prevention Plan in collaboration with each assigned member and their team
- Be knowledgeable about behavioral health care services, system partners, service continuum and established medical necessity criteria and apply that knowledge in all work activities.
- Attend up to 3 hours of group or individual skills-based coaching/supervision per week
- Complete Utilization Management activities including but not limited to: interagency meetings, social determinants interviews with members, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage, within established timeframes.
- Authorize treatment when Medical Necessity Criteria is met.
- Consult with Physician Advisor, Psychologist Advisor and/or Clinical Leadership as clinically appropriate.
- Generate denial letters based on physician reviews and in accordance with state regulations as needed.
- Demonstrate an understanding of Evidence-Based Practices (EBPs) and connect members to appropriate evidence-based treatment.
- Collaborate with other CBH departments to ensure quality of care, promote placement of members in the most appropriate services and to facilitate consistent longitudinal care management.
- Document provider quality concerns in accordance with CBH policy/procedure.
- Use data to inform clinical decision-making.
- Maintain documentation in CBH’s electronic system consistent with agency and industry standards.
- Respond to care coordination correspondence within 24-48 hours or sooner if necessary.
- Attend all meetings as assigned and actively prepare for and participate in supervision and clinical case consultations.
- Ensure the highest quality of customer service when interacting with members, providers, and other stakeholders.
- Display commitment to member/family-driven care and principles of resiliency and recovery.
- Contribute to projects and other duties as assigned.
Team Specific Responsibilities:
- Facilitate monthly interagency HFW Planning Meetings (includes youth/family participation) for a caseload of approximately 10 members and their families.
- Write, revise and distribute individualized Wraparound Plans and Discovery documents to member’s teams monthly.
- Ensure that the principles and steps of the High - Fidelity Wraparound process are delivered with the highest possible fidelity.
- Provide evidence-based intensive care management interventions directly to members in the community.
- Maintain a care management caseload of approximately 10 members (or as otherwise directed).
- Facilitate care coordination with external entities including behavioral and physical health providers, Child Welfare, Juvenile Justice, School district of Philadelphia, and Targeted Case Management (TCM), as well as any other providers or systems involved with assigned members’ families.
- Address member/caregiver engagement in treatment by reporting or addressing any reported/observed quality concerns, resolving communication barriers between member (and/or caregiver) and providers, assisting member/caregiver with mediation issues/barriers, assisting with any clarification/prioritizing issues, increase positive collaboration with providers and other supports.
- Facilitate longitudinal care management of all assigned members.
- Complete UM activities as assigned and within required timeframe
- Participate in and complete training and credentialing in the High-Fidelity Wrapround model of care management.
- Maintain a flexible schedule, which may include some evenings or occasional weekends, in order to accommodate the scheduling needs of assigned members and their families
Position Requirements:
- Education: Master’s degree in Social Work or related fieldClinical Licensure: Qualified candidates are required to obtain license within the first 18 months (1.5 years) of employment; PA Child Abuse Clearance required
- Required Licensure: Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW)
- Must obtain High - Fidelity Wraparound Facilitator credential within 1 year of hire.
- Relevant Work Experience: Minimum of 3 years of direct clinical care experience working with adults, children, adolescents and/or their families in behavioral health settings and experience in assessment and treatment planning; and/or experience in a managed care setting.

Uncommon Middle School History teachers work in grade level teams and departments to push students towards being self-guided learners. Our teachers develop subject mastery and have a keen sense of the socio-emotional changes middle school students experience. History classes aim to empower students' voices and develop them into global citizens by emphasizing textual analysis, analytical writing, and discourse. Teachers also use engaging instructional strategies to make history come alive! At the conclusion of eighth grade, students are prepared for the academic autonomy and college preparatory focus of our high schools.

High School Teachers hold the primary responsibility for the implementation and development of Uncommon’s curriculum and the success of its students. Therefore, Uncommon Schools seeks teachers who are committed to continuously improving curriculum and instruction through collaboration as part of a grade-level and school team. Visual Arts:
We are seeking a high school Visual Arts Teacher to lead basic, intermediate, and advanced art courses for teenage students. This role will oversee instruction across multiple fine art disciplines, including drawing, painting, sculpture, ceramics, and mixed media, with opportunities for students to develop a cohesive portfolio of work over time. Courses will balance technical skill-building, creative expression, and critical analysis, while integrating art history, contemporary art practices, and visual literacy.

Instruction:
You’ll teach Visual Arts content across multiple fine art mediums, including drawing, painting, sculpture, ceramics, and mixed media, with opportunities for advanced exploration and specialization.
You’ll design and implement engaging, developmentally appropriate lessons that build technical skills, artistic voice, and creative risk-taking.
You’ll design coursework that supports portfolio development and prepares students for advanced study, exhibitions, or postsecondary pathways in the arts.
You’ll integrate art history, cultural context, and contemporary artists into instruction to strengthen critical thinking and visual analysis skills.
You’ll implement curricula and learning activities aligned to academic standards and Uncommon’s instructional expectations.
You’ll design and administer formative and summative assessments to measure student growth and mastery of artistic skills and concepts.
You’ll set assessment data and student work analysis to refine curriculum, differentiate instruction, and inform instructional practices.
You’ll participate actively in collaborative curriculum development, grade-level planning, and school-wide initiatives.
You’ll establish and maintain a structured, inclusive classroom environment with consistent rewards and consequences aligned to school expectations.
You’ll hold high academic and artistic expectations and take ownership of student progress and outcomes.
School Culture and Daily School Activities:
You’ll build strong, positive relationships with students to ensure they feel seen, valued, and supported as artists and learners.
You’ll partner with students’ families to share progress, celebrate student work, and ensure appropriate resources are available to support learning.
You’ll engage in practice-based professional development, mentorship, coaching, and reflective practice to continuously improve instruction.
You’ll support school-wide responsibilities including morning arrival, lunch duty, student assemblies, class transitions, and after-school dismissal.
Qualifications
A demonstrated commitment to supporting students’ social-emotional and academic development. An enthusiasm for working with families in the best interest of students. A self-directed learner who solicits and implements feedback to improve outcomes and achieve objectives.
Required Experience
- A demonstrated interest in impacting K–12 students in urban schools and communities (1–2 years experience preferred).
- Academic expertise in early childhood education.
- Prior to the start of employment, you must have a bachelor's degree from an accredited college or university.

At Uncommon, our College Counselors work closely with students and parents to find the right colleges and universities to match their goals and enable them to achieve their dreams. College Counselors ensure aligned college matches for students regarding school community, size, location, student to faculty ratio, academic interests, financial aid, and extracurricular interests. The ultimate goal of the College Counselor is to ensure that each student graduates ready to thrive in college and beyond.
Responsibilities:
- You'll teach 11th and 12th grade college seminar courses in which you will guide students through the entire college application process. This includes (but is not limited to) exploring careers, identifying most appropriate post-secondary pathways, writing personal statements, building resumes, securing letters of recommendation, and applying for financial aid and scholarships.
- You'll build a culture of college and career readiness amongst all students by creating inspirational and instructional signage, advising students in the course selection process, and helping plan and execute events such as Career Day, SAT School Day, Senior Signing Day, College Acceptance Forums, College Visits and Alumni Day.
- You'll collaborate with parents and families to ensure a positive partnership by providing informational workshops and hosting individualized meetings.
- You'll continually refine your expertise by participating in Uncommon PD and attending regional and national conferences.
- You'll build relationships with college admissions offices and advocate relentlessly on behalf of your students to facilitate the admissions and financial aid process.

Summary:
- The Patient Services Representative II (PSR II) represents the face of our company to patients who come in, both as part of their health routine or for insights into life-defining health decisions.
- The PSR II draws quality blood samples from patients and prepares those specimens for lab testing while following established practices and procedures.
- The PSR II has direct contact with patients and creates an atmosphere of trust and confidence while explaining procedures to patients and drawing blood specimens in a skillful, safe and accurate manner.

Key Responsibilities:
- Maintain equipment inventory, ensuring the information is accurate and updated promptly.
- Regularly evaluate fixed lab equipment (fume hoods, exhaust fans, biosafety cabinet, etc) are functioning properly.
- Manage all laboratory vendor service contracts ensuring that preventative maintenance visits are executed on schedule
- Manage and coordinate timelines across laboratories for certifications and calibrations on critical equipment
- Help resolve and/or coordinate equipment repair visit between the service provider and the laboratory team.
- Review cold storage monitoring software to minimize the risk of freezer failure. Escalate issues internally and troubleshoot as needed.
- Participate in laboratory emergency planning/management - e.g. power outages and infrastructure projects
- Work with all parties involved (technicians, users, IT, and leadership) to ensure that both the equipment and operational needs are met.
- Chaperone technicians from external suppliers and ensure they are adhering to company policies while onsite.
- Oversee delivery logistics by coordinating the receipt of packages, gas deliveries, or installation of laboratory equipment.
- Support the procurement of laboratory consumables and supplies as needed.
- Perform general troubleshooting of laboratory operation issues as they arise.
- Support the development and maintenance of procedures to ensure compliant lab operations.
- Work with lab safety officer to manage the safety systems/compliance and ensure the Personal Protective Equipment (PPE) is current and available.
- Provide miscellaneous laboratory maintenance tasks as needed e.g. supply stocking, eye wash/safety shower flushing, removal of waste, etc
Required Qualifications:
- A.S. or B.S. in Life Sciences or Facilities Management with management/technical emphasis.
- Minimum 1 year of pharmaceutical, life sciences, or biotech experience in an academic or industry setting.
- Minimum 2 years of experience implementing maintenance and repair programs in a Research & Development (R&D) facility.
- Strong interpersonal skills with the ability to work collaboratively with member of cross-functional team
