POSITION PURPOSE

Provides and directs safe, effective, and culturally competent care for patients with acute medical and surgical conditions.   • Makes an initial assessment of new admissions within 30 minutes of the patient’s arrival to the unit.
  • Completes all areas of the admission assessment database within 24 hours of admission.

2. Identifies patient problems and develops them into an outcome/goal-based plan of care.

  • Initiates the patient problem list and incorporates relevant medical history into the plan of care.
  • Analyzes and interprets the assessment data to identify desired patient outcomes/goals.
  • Determines nursing interventions to achieve those outcomes/goals.
  • Prioritizes patient care needs/ activities.
  • Identifies and makes appropriate referrals (i.e., Patient/Family Services, Nutritional Services, Spiritual Care, Diabetic Educator, etc.), and education needs.
  • Collaborates with other health care disciplines to plan interventions/ patient outcomes/goals.

3. Implements the outcome/goal- based plan of care.

  • Assigns and coordinates patient care incorporating patient acuity and the skills, knowledge and abilities of the team members.
  • Organizes and prioritizes care for the patients within the team.
  • Delegates tasks appropriately to other team members.
  • Implements the plan of care including procedures, treatments, physicians orders, nursing orders, education plan and medications.

  • Evaluates patient response(s) to interventions.
  • Review/modifies/revises the outcome-based plan of care based on ongoing reassessments.
  • Updates the problem list and outcomes/goals.

5. Communicates relevant clinical information to the team to optimize patient outcomes/goals.

  • Communicates relevant clinical information to physicians regarding the patient’s condition.
  • Reports progress toward outcomes/goals and/or changes in condition to other health team members.
  • Maintains ongoing communication with interdisciplinary team members throughout the shift, at shift change, and transfer of care.
  • Communicates with other team leaders and members throughout the shift to reassess and prioritize the needs of the unit.
  • Documents completely, comprehensively and legibly and in accordance with policy.

6. Demonstrates clinical competence.

  • Utilizes Patient Care Policies and Procedure as references to guide nursing practice.
  • Assures medication safety by following the “5 Rights” of medication administration.
  • Prioritizes and implements intervention according to patient assessment.
  • Demonstrates competency in computer systems and applications required for role.
  • Receives, transcribes and processes and implements physician orders per policy including dating, timing and verifying accuracy.

This position will assume responsibility for verifying that appropriate pre-certification documentation is available to ensure reimbursement, quoting rates for therapy within established guidelines, and coordinating care with applicable agencies and the Mount Carmel Infusion Services pharmacy staff. This person will function with a high level of independent judgment, using skills and knowledge acquired through education and customer service expectations to enhance patient care. In addition, the nurse will contribute clinical expertise to organizational planning and decision-making and may also be involved in business development activities as they relate to business expansion or new program development. 

  

What You Will Do: 

  • Maintains daily communication with case managers/discharge planners and units to help ensure that patients referred for services are followed up with in a timely and efficient manner. 

  • Responsible for the successful clinical coordination of care from the referral source into the home setting on behalf of Mount Carmel Infusion Services. 

  • Provides and utilizes clinical expertise and skills to assist referral sources &/or patients with: 

  • The identification of appropriate candidates for home infusion &/or home health care 

  • The identification of the patients’ and/or caregivers ability to provide care 

  • Patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home. 

  • Medication connection at the hospital / facility in order to expedite the discharge into the home setting, as appropriate 

  • Coordinates and communicates all discharge planning activities for new patient referrals with Mount Carmel Infusion Services personnel and referral sources. 

  • Appropriately documents the coordination of care activity including, but not limited to: 

  • Home infusion &/or nursing services utilized 

  • All communication with patients, referral sources, physicians and Mount Carmel Infusion Services personnel 

  • All patient &/or caregiver education & training 

  • Interacts with healthcare team members to answer any questions related to home infusion and community-based services as they plan for patient discharges from the acute care setting. 

  • Functions as a liaison between nursing and other disciplines and departments in resolving patient issues. 

  

Minimum Qualifications: 

  • Education: Bachelor’s degree in nursing (BSN) or Registered Nurse (RN) from an accredited college or school of nursing with equivalent experience required 

  • License / Certification: Current license to practice as a Registered Nurse in the State of Ohio required. 

  • Current Certified Registered Nurse Infusion preferred 

  • (CRNI) credential preferred or be willing to obtain within 6 months of hire 

  • Must have valid and current Ohio Driver’s license and reliable transportation to and from work site
