The Therapist II Engage PT/PRN works both independently and as a member of a team. The incumbent also works as part of a coordinated specialty care team for youth and young adults who are experiencing FEP. The position has the ability to support family members through ongoing collaboration, coordination with schools, colleges, and universities, as well as organizations and community groups outside of behavioral health.

Maintains familiarity with the FEP CT-R model of therapy through HeadsUp. Utilizes other approaches, such as CBT, MI, etc.
Uses a variety of educational materials, brief intervention techniques, and community resources to engage and increase consumer motivation, as well as to manage their chronic behavioral and medical conditions.
Maintains understanding of community resources to support consumers in achieving their goals.
Promotes shared decision-making when developing treatment plans and assessing progress towards goals.
Establishes a relapse prevention plan and a follow-up schedule with the consumers, monitoring consumers progress throughout the maintenance phase.
Systematically screens consumers for depression and unhealthy substance use by utilizing evidence-based screening instruments appropriate to the population.
Clearly and effectively communicates with other providers, including both medical and behavioral health. In particular, works closely with inpatient settings when a consumer enters that level of care and works with the facility treatment team around discharge planning.

Position Overview:

The purpose of this position is to serve in the Registered Nurse and Director of Nursing capacity. Essential Job Functions:
  • Responsible for covering a minimum of three 12-hour clinical Registered Nurse shifts weekly per Hospital Administrator direction, to include:
  • Plan, prioritize, and provide proficient, age-specific and developmentally/specialty appropriate nursing care by applying advanced specialty clinical nursing knowledge and skills and for ensuring the comfort and safety of patients/families
  • Proficiently and consistently complete accurate, concise and legible documentation on medical records
  • Observe and record signs, symptoms and behaviors including the physiological status of patients
  • Presents proposed revision of interventions and desired outcomes to the physician
  • Maintain patient's privacy and confidentiality of information and records at all times
  • Follow physician’s orders in the delivery of nursing care
  • Collect laboratory samples and perform lab testing in accordance with established policies and procedures, as well as COLA and CLIA regulations

Key Responsibilities Perform emergency medical treatment for Sailors, Marines, and specialized units such as SEALs and Seabees; provide basic and emergency dental care and process dental X rays; serve as operating room technician and assist in surgery; administer preventive care and medications including immunizations and IVs; conduct physical exams and assist in diagnosing and treating diseases and injuries; maintain patient records, perform clinical tests, and support physicians and nurses in a wide range of specialties.

Position Overview:

The purpose of this position is to perform a variety of imaging procedures, including CT and X-Rays, at a technical level not requiring constant supervision. 

Essential Job Functions:
  • Provide technical assistance and supportive patient care to assist the physicians, nurses and other technical and administrative staff in meeting the needs of individual patients throughout the facility.
  • Operate and maintain all imaging department equipment including but not limited to X-Ray, CT, CR reader, and PACS
  • Perform all imaging exams in accordance with established policies, procedures, regulations, and laws and ensure the physician on duty is notified of the results
  • Perform basic clinical procedures under the direction of the physician and/or nurse on duty
  • Maintain all required documentation, logs, charts, forms and records in paper and electronic formats
  • Collect laboratory samples and perform lab testing in accordance with established policies and procedures, as well as COLA and CLIA regulations
  • Maintain an adequate supply of all reagents and consumables to perform quality testing
  • Perform all routine daily, weekly, monthly, and periodic maintenance and function checks following established protocol for all imaging equipment
  • Perform Quality Control as established by this laboratory
  • Perform Proficiency Testing and/or Split Sample analysis periodically as established by this facility
  • Retain records of all analytic activities performed for a minimum of two years
  • Perform all Quality Assessment activities assigned to this position and document these activities for periodic review by the Laboratory Supervisor and/or the Laboratory Director
Other Job Functions:
  • Perform duties as ER Technician as needed

Basic Qualifications:

  • Certified as a medical radiologic technologist as required by the state in which practicing required
  • Registered Technologist with the American Registry of Radiologic Technologists (ARRT) in radiography required
  • Registered Technologist with the American Registry of Radiologic Technologists (ARRT) in computed tomography, preferred, with the following exceptions
    • CT certification required (Las Vegas Market)
    • CT certification required within 12 months of hire (Pennsylvania market)
  • Current BLS certification through the American Heart Association, required
  • 2+ years radiologic/imaging experience, preferred
  • 1+ years of CT experience, preferred

The CHS Evaluation & Education Support Coordinator is passionate about working with teachers and interventionists to ensure all children have their needs met. This individual is invested in the health and wellbeing of young children across developmental domains including physical, cognitive, language, social & emotional and adaptive development at CHS. They will lead the early intervention/identification and evaluation process and will support instructional staff in identifying and implementing supports for behavioral and developmental concerns. This individual will also oversee efforts to understand the medical and developmental history of each student as well as assisting with any behavioral or developmental concerns, including completing a plan to recommend relevant interventions when necessary. They will lead the Center Child Study team in situations which require additional intervention and will be responsible for collecting and organizing data and information to inform referrals and work with child study team to recommend the next steps for the child. The Evaluation & Education Support Coordinator will report to the Health Services Manager. The salary range for this position is $60,000 to $69,344 per year, based on expertise.

Responsibilities:

* Understanding the most updated standards of child assessment and evaluation, and planning and coordinating screening and evaluation schedules with teachers, families, or sponsors to support child needs
* Assisting in the evaluation process for behavioral concerns and serve as a resource to staff by disseminating research regarding intervention tools and techniques to all who work with the child
* Observing and recording behavioral concerns for identified children and working with Teachers to document behavior
* Researching and recommending tools to support children's access to the curriculum who have developmental delays and other special rights. Organizing data of relevant children for further assessment
* Requesting and obtaining medical records in partnership with the Center Nurse for each child under evaluation
* Researching and referring children to the correct programs and services to address their needs. Gathering any additional evaluations based on IFSP and IEP goals and filling out associated forms
* Ensuring that the proper procedure is followed for children that require Speech, Occupational, and Physical therapies
* Organizing schedules of early intervention providers who are coming to the Center to provide services/supports
* Coordinating transportation for services that do not occur at the Center
* Connecting families to additional services when children have needs that do not qualify them for early intervention services
* Providing a monthly report to the Health Services Manager of all data related to intervention services
* Leading Child Study Team Meetings, and developing and recording child-specific support plans in collaboration with the Child Study Team
* Planning training and workshops for teachers and parents to support children with special rights in collaboration with the Assistant Director and Instructional Coaches
* Working with Assistant Director to develop staff training related to special needs and exceptionalities
* Assisting with transition meetings of children with early intervention plans, and acquiring consent forms from parents/primary caregivers
* Collaborating with the nurse, to obtain relevant child information from parents/primary caregivers to understand the child's full medical history
* Acting as a mentor, in coordination with the Assistant Director, to teachers as they learn and employ intervention strategies and tools
* Maintain accurate and updated records that reflect progress and development of children and the process of obtaining recommended interventions where needed

Qualifications:

* Bachelor's Degree in Early Childhood Education, Special Education, Early Intervention, Child Development, or a related field. Master's Degree in related field preferred
* 5 or more years of experience in early intervention
* Experience interpreting writings and implementing IFSPs and IEPs (preferred)
* Exceptional executive functioning, organizational, and time management skills, and an action-oriented mindset
* Knowledge and understanding of proper procedure for children that require Speech, Occupational, and Physical therapies
* Ability to work in a fast-paced environment with changing priorities
* Responsible and enthusiastic, cares about working in a mission-focused organization
* Demonstrated understanding of and ability to effectively work with low-income families (preferred)
* Ability to proactively identify problems and implement solutions
