Responsibilities and Duties
- Manage a caseload of 3–5 patients on service, including consults
- Oversee dialysis for 5–6 patients at a time
- Participate in kidney transplant care coordination (transplants performed at a different system)
- Follow up with a panel of transplant patients (currently 40, with growth expected)
- Support outpatient services, with a target of 3500 visits per year as the program develops
- Respond to after-hours patient calls; rare in-hospital call-backs
- Ensure 30-minute response time for calls and patient inquiries
- Collaborate effectively with a multidisciplinary team
- Demonstrate strong bedside manner and clear communication with patients
