ChristianaCare is seeking a detail‑oriented and patient‑focused Medication History Technician to join our team. In this critical role, you will collect and confirm precise home medication details shared by patients and their caregivers, helping ensure medication safety from the moment of admission. Your work will directly support pharmacists and physicians in delivering high‑quality, coordinated care.
This position is full time day/evening shift rotation. Every other weekend required; holidays as required.
Principal Duties and Responsibilities:
- Interview patients, caregivers, and other sources to acquire accurate and complete home medication histories.
- Ask clarifying questions regarding medication names, dosages, frequency, and last used to ensure accurate data collection.
- Enter collected information into the EMR following established procedures to produce admission medication reconciliation documentation.
- Generate medication history reports for pharmacist review and subsequent physician use.
- Collaborate closely with inpatient pharmacy staff and other care providers to support medication‑related needs.
- Navigate the ED tracking board to determine appropriate patient selection.
- Update or build medication records within the EMR with accuracy and attention to detail.
- Apply good clinical judgment when assessing medication lists; identify inconsistencies or concerns and communicate them to the pharmacist.
- Verify medication information using external sources when necessary (pharmacies, provider offices, etc.).
- Coordinate with charge nurses and patient nurses to determine optimal times for patient interviews.
- Demonstrate strong knowledge of both brand and generic medications, common dosages, dosage forms, and typical administration schedules.
- Communicate effectively with patients of all education levels in a fast‑paced Emergency Department setting.
- Assess the completeness of medication lists, including drug name, dose, frequency, indication, and time of last dose.
- Acquire accurate histories of PRN medications, including frequency and indications.
- Determine patient medication adherence prior to admission.
- Build rapport with patients and families to acquire reliable histories or identify alternative information sources when needed.
- Prompt patients for overlooked medications, including over-the-counter items such as herbals, vitamins, pain relievers, cold remedies, inhalers, patches, creams, eye drops, and more.

This is an entry level driver training role. This driver is responsible for learning to and/or gaining experience in driving a tractor trailer or straight truck intrastate and/or interstate on local, over-the-toad (OTR), shuttle, and/or overnight routes to deliver and unload various food and food related products to customers. All routes are expected to be completed safely in accordance with all Company policies and Department of Transportation (DOT) regulations. The Driver Trainee communicates and interacts with customers, vendors and co-workers professionally ensuring all services and duties are executed in accordance with preferred work methods and customer service practices. Functions as a team member within the department and organization, as required, and performs any duty assigned to best serve the company.
Position Responsibilities:
- Attends and successfully completes PFG Entry Level Driver Trainee or Dock to Driver Training Program as required. All training documentation is completed and maintained per requirements.
- Rides-with and assists driver trainer in executing deliveries as required. Follow all instructions and directions provided by driver trainer.
- Perform all required safety checks (i.e. pre/post trip) including inspections of tractor/truck and trailer according to Department of Transportation (DOT) regulations; inspect tractor/truck and trailer to insure they meet company safety standards and take appropriate action as needed. Report all safety issues and/or repairs required.
- Follow all DOT regulations and company safe driving guidelines and policies. Immediately report any and all safety hazards.
- Inspect trailer for properly loaded and secured freight. Perform count check of items and check customer invoices of products that have been loaded. Check and complete in an accurate and in legible fashion all required paperwork associated with freight. Move tractor to the loading dock and attach preloaded trailer as needed.
- Drive to and deliver customer orders according to predetermined route delivery schedule.
- Unload products from the trailer, transport items into designated customer storage areas. Perform damage control checks on items, scanning and contact supervisor about removing orders according to company policy. Verify delivery of items with customer and obtain proper signatures. Collect money (cash or checks) where required. Load customer returns on to trailer and secure trailer doors.
- Ensure that tractor, trailer and freight are appropriately locked and/or secured at all times.
- Unload damaged goods and customer returns and bring to the driver check-in and complete necessary paperwork. Unload all equipment, materials and remove trash from trailers as required.
- Complete daily record of hours of service and enter in log in accordance with Federal DOT, state and company requirements.
- Perform general housekeeping duties in tractor, loading dock area and keep trailers clear and clean as required. At the end of the shift secure all equipment and complete all necessary paperwork.
- Performs other related duties as assigned.

- Assign project IDs, assemble internal project teams, and communicate project timelines.
- This includes building relevant internal teams for each program and creation of entries in the multiple project planning tools that drive the work at the lab organized by molecule. Notifications from the various tools inform the testing and support teams of upcoming demand.
- This includes notifying the relevant planning/scheduling staff and test material coordination teams to clarify planning for specific testing needs.
- Work directly with Group Leaders, SMEs, Study Monitors and Strategic Outsourcing to initiate studies to be placed at contract labs, ensuring timeline deliverables are met.
- Maintain master list of studies and update study initiation statuses with relevant milestones.
- Once study schedules are finalized, each study milestone date is entered into the Milestone-Tracking SmartSheet that enables the testing teams and the support teams (scientific writing and quality assurance) to anticipate demand and effectively prioritize across programs.
- Ensuring consistency and accuracy of information in wider Regulatory and Stewardship planning tools as the details are translated and maintained in the local project management tools in use at the lab that permit a necessary higher level of detail to enable completion of work product at the lab that is in alignment with the overall program needs and to ensure the accuracy of the GLP Master Schedule.
- Maintain internal study costing data across multiple disciplines.

Job Requirements:
- B.A. or B.S. degree or equivalent relevant experience.
- Demonstrate expertise in project management in a laboratory setting.
- Design and implement process/workflow upgrades which impact project or competency objectives.
